医院主导的家庭心脏康复在经皮冠状动脉介入术后老年患者中的应用

Hospital-led family cardiac rehabilitation in percutaneous PCI application effect in post-elderly patient

目的探讨医院主导的家庭心脏康复模式对老年急性心肌梗死患者行经皮冠状动脉介入治疗术后心功能状态、血脂水平及自我管理能力的影响。方法选取2020年1-10月由我院急诊科120转运队接诊住院行经皮冠状动脉介入治疗术的120例老年急性心肌梗死患者作为研究对象,随机分为干预组和对照组各60例。对照组给予心内科常规护理,干预组实施医院主导的家庭心脏康复护理。比较2组患者在干预前及干预后12周心功能指标、血脂水平、SAQ生活质量评分及心血管不良事件发生率。结果干预后2组的左心室收缩末期容积(LVESV)和左心室舒张末期容积(LVEDV)结果比较差异无统计学意义,但干预后左心室射血分数(LVEF)明显高于干预前(P<0.05),且干预组高于对照组。干预后2组TG、总胆固醇水平比干预前有所下降(P<0.05),且干预组优于对照组。干预后干预组SAQ各项评分均高于对照组(P<0.05)。干预组再发心绞痛、心肌梗死及心血管不良事件的发生率低于对照组(P<0.05)。结论医院主导的家庭心脏康复可有效改善急性心肌梗死经皮冠状动脉介入治疗术后患者的心功能状况、降低血脂水平、提高患者家庭康复运动的参与度和自我管理能力,促进快速康复。

Objective To investigate the influence of CHBCR model on PCI postoperative cardiac function,blood lipid and self-management ability in elderly AMI patients.Methods Selected 110 elderly AMI patients(the control group:52 samples,the study group:58 samples)with PCI surgery from the 120 transfer team of our emergency department from January to October 2020.Routine care of cardiology department in the control group,the study group performed CHBCR-mode cardiac rehabilitation care.The incidence of cardiac function indicators,blood lipid levels,quality of life,and adverse cardiovascular events after both groups were compared.Results The two groups of LVESV、LVEDV had no differences.The LVEF was significantly higher than that before the intervention and the study group was higher than the control group.Cholesterol levels in both groups compared with prior intervention and the study group was superior to the control group.The SAQ scores were higher than that in the control group.The rates of recurrent angina,myocardial infarction,and adverse cardiovascular events were lower than in the control group.Conclusion Cardiac rehabilitation nursing based on CHBCR mode can effectively improve the cardiac function after PCI,reduce blood lipid,and improve the participation and self-management ability in elderly AMI patients.
