门诊处方审核系统改进与效果分析

Improvement and Effect Analysis of Outpatient Prescription Review System

目的探讨六西格玛管理在门诊处方审核中的应用。方法运用六西格玛管理中的定义、测量、分析、改进、控制5个步骤,采用因果矩阵、潜在失效模式与后果分析等质量管理工具,对门诊处方审核流程与关键环节进行分析,制定相应的改进措施,并对改进前后门诊处方审核效果进行分析。结果改进后,门诊处方不合格率从9.1‰下降至3.2‰,流程能力Z值从2.36提高至2.71,改进前后差异有统计学意义(p<0.05)。结论六西格玛管理能够改进门诊处方审核效果,保障用药安全。

Objective To explore the application of Six Sigma management in inspection on prescriptions in outpatient department.Methods Quality management tools such as causality matrix,potential failure mode and consequence analysis were used to analyze the process and key links of the inspection on prescriptions in outpatient department,which was following the five steps of definition,measurement,analysis,improvement,and control in Six Sigma management.Corresponding improvement measures were formulated,and the inspection effect of outpatient prescriptions before and after the improvement were analyzed.Results After the improvement,the unqualified rate of outpatient prescriptions decreased from 9.1‰to 3.2‰,and the Z value of the process ability increased from 2.36 to 2.71.The difference before and after the improvement was statistically significant(p<0.05).Conclusion Six Sigma management can improve the effect of outpatient prescription review and ensure the safety of medication.
