解郁安神方联合盐酸舍曲林治疗持续性姿势知觉性头晕患者临床研究

Therapeutic Effect of Jieyu Anshen Decoction Combined with Sertraline Hydrochloride on Patients with Persistent Postural Perceptual Dizziness

目的:观察解郁安神方联合盐酸舍曲林治疗持续性姿势知觉性头晕(PPPD)患者临床研究。方法:选择60例PPPD患者随机分为对照组(28例,脱落2例)、观察组(27例,脱落3例)。对照组给予口服盐酸舍曲林片(每次50mg,qd)治疗;观察组在对照组基础上,加用自拟解郁安神方(1剂/d);2组均治疗4周为一疗程,比较2组治疗前后的眩晕残障量表(Dizziness Handicap Inventory,DHI)评分、汉密尔顿焦虑量表评分(Hamilton Anxiety Scale,HAMA)、汉密尔顿抑郁量表评分(Hamilton Depression Scale,HAMD)及不良反应发生情况,并评定临床疗效。结果:对照组有效率为67.86%,观察组有效率为92.59%,观察组疗效明显优于对照组,差异有统计学意义(P<0.05)。2组患者治疗后DHI评分及HAMA评分、HAMD评分均较治疗前降低,差异具有统计学意义(P<0.05);观察组各项指标较对照组均明显改善(P<0.05);同时观察组安全性更好。结论:解郁安神方联合盐酸舍曲林治疗持续性姿势知觉性头晕(PPPD)的疗效显著,可改善患者头晕、焦虑、抑郁等临床症状,优于单纯使用盐酸舍曲林治疗。

Objective:To observe the clinical study of Jieyu Anshen Decoction combined with Sertraline Hydrochloride in the treatment of patients with persistent postural perceptual dizziness (PPPD).Methods:Sixty cases of PPPD patients were randomly divided into control group (28 cases,2 cases dropped) and observation group (27 cases,3 cases dropped).The control group was given oral sertraline hydrochloride tablets (50 mg each time,qd);the observation group was treated with self-made Jieyu Anshen Recipe (1 dose/d) on the basis of the control group;both groups were treated for 4 weeks as a course of treatment,Compare the Dizziness Handicap Inventory (DHI) score,Hamilton Anxiety Scale (HAMA),Hamilton Depression Scale (HAMD) and adverse reactions before and after treatment in the two groups,And evaluate the clinical efficacy.Results:The effective rate of the control group was 67.86%,the effective rate of the observation group was 92.59%,the observation group was significantly better than the control group,and the difference was statistically significant (P<0.05).The DHI scores,HAMA scores,and HAMD scores of the two groups of patients after treatment were all lower than before treatment,and the difference was statistically significant (P<0.05);all indicators of the observation group were significantly improved compared with the control group (P<0.05);at the same time,the observation group Security is better.Conclusion:Jieyu Anshen Decoction combined with Sertraline Hydrochloride has a significant effect on persistent postural perceptual dizziness (PPPD).It can improve patients’ dizziness,anxiety,depression and other clinical symptoms,and is better than using Sertraline Hydrochloride alone.
