摘要
目的分析人血浆脂蛋白相关磷脂酶A2(Lp-PLA2)、同型半胱氨酸(Hcy)、血脂水平与不同肥胖类型的关系。方法选取2018年7至12月中国人民解放军空军杭州特勤疗养中心疗养一区的体检人群474例,检测其血清Hcy、Lp-PLA2、TC、TG、HDL-C及LDL-C水平。男、女性根据BMI分为对照组(男112例,女132例)、超重组(男125例,女51例)和肥胖组(男39例,女15例);按腰围分为腰围正常组(男138例,女139例)和中心性肥胖组(男138例,女59例),分析并比较各项指标在不同性别BMI及腰围各亚组中的差异,以及BMI和腰围与不同性别各项指标的相关性。结果男性组和女性组肥胖率分别为14.1%(39/276)和7.6%(15/198),中心性肥胖率分别为50.0%(138/276)和29.8%(59/198),两组肥胖率、中心性肥胖率差异均有统计学意义(均P<0.05)。男性和女性中心性肥胖组的TG和HDL-C水平与腰围正常组比较差异均有统计学意义(均P<0.01);男性中心性肥胖组Lp-PLA2水平高于腰围正常组(P<0.05);女性中心性肥胖组Hcy水平高于腰围正常组(P<0.05)。在男性中,BMI和腰围与TG均呈正相关(均P<0.01),而与HDL-C水平呈负相关(P<0.01)。在女性中,BMI与Hcy和TG均呈正相关(均P<0.05),而与HDL-C呈负相关(P<0.01),腰围与Hcy、TG和LDL-C均呈正相关(均P<0.05),腰围与HDL-C呈负相关(P<0.01)。结论肥胖特别是中心性肥胖人群中患心血管疾病和代谢性疾病的危险较高,故应针对性开展健康教育,提高人群健康意识,降低因肥胖特别是中心性肥胖引起的疾病风险。
Objective To analysis the relationship of obesity with plasma lipoprotein associated phospholipase A2(Lp-PLA2),homocysteine(Hcy)and lipids levels.Methods A total of 474 health subjects undergoing check-up in Air Force Health Care Center for Special Services Area 1 from July to December 2018 were enrolled in the study.The height,weight,waist circumference,plasma Lp-PLA2,Hcy,TC,TG and HDL-C levels were measured.According to BMI levels,there were 244 normal body weight subjects(112 males and 132 females,control group),176 overweight subjects(125 males and 51 females,overweight group)and 54 obese subjects(39 males and 15 females,obesity group),according to waist circumference(WC),there were 277 subjects(138 males and 139 females)in the normal WC group and 197 subjects(138 males and 59 females)in the central obesity group.Results The prevalence rates of obesity in male group and female group were 14.1%and 7.6%,respectively(P<0.05).The rates of central obesity in male group and female group were 50.0%and 29.8%,respectively(P<0.05).The TG and HDL-C levels in the male and female central obesity group were significantly higher than those in normal WC group(P<0.01).The Lp-PLA2 level in the male central obesity group was higher than that in the normal WC group(P<0.05).The Hcy level in the female central obesity group was higher than that in the normal WC group(P<0.05).BMI and WC were positively correlated with TG(P<0.01),and negatively correlated with HDL-C in males(P<0.01).BMI was positively correlated with Hcy and TG(P<0.05),and negatively correlated with HDL-C level in females(P<0.01).WC was positively correlated with Hcy,TG and LDL-C(all P<0.05),and negatively correlated with HDL-C in females(P<0.01).Conclusion Health education should be strengthened to raise health awareness and to reduce the risk of obesity related diseases.
