急诊危重症患者院间转运现状的横断面调查研究

A cross-sectional survey of the current status of interhospital transfer for critically ill patients

目的了解急诊危重症患者院间转运现状,以期为患者院间转运相关工作规范的制订提供参考。方法2021年4月6日至2021年4月26日,采用便利取样法,以网络问卷调查的形式,对全国26个省、直辖市的97家医疗机构的2142名急诊医护人员进行急诊危重症患者院间转运现状调查。结果急诊医护人员认为危重症患者院间转运存在的挑战前三项分别为转运前后的病情评估、转运相关的沟通、转运前后的病情交接;最担心的潜在事件前三项分别为患者病情变化、发生医疗纠纷、转运设备不足或故障。院间转运实施难易程度的平均分为(6.40±2.01)分,院间转运通畅度的平均分为(8.00±1.85)分,101名(9.6%)医护人员参与院间转运发生过的相关事件。结论我国急诊危重症患者院间转运工作相关流程有待规范,需逐步完善院间转运流程、院间转运信息平台、院间转运监测网的建设,以促进医疗单位间的有效沟通,保障院间转运患者安全。

Objective To investigate the current status of interhospital transfer of critically ill patients in order to provide reference for the formulation of relevant work specifications for patients′interhospital transfer.Methods Convenient samples from emergency medical staff were collected through internet survey in 97 hospitals of 26 provinces and municipalities directly under the central government in China from April 6,2021 to April 26,2021.2142 valid questionnaires were collected totally.Results Medical staff believed that the top three challenges of interhospital transfer were:condition assessment,communication,and handover before and after transfer.The top three worried potential events were:the changes of patient′s condition,the occurrence of medical disputes,insufficient or malfunctioning of transfer equipment.The average score for the ease of implementation of interhospital transfer was(6.40±2.01)scores,and the transfer patency was(8.00±1.85)scores.101(9.6%)medical staff participated in the interhospital transfer related events.Conclusions There is still room for the improvement about interhospital transfer in China.It′s necessary to improve the construction of the interhospital stransfer process,the information platform,and the monitoring network to promote effective communication between medical units and ensure the transfer safety.
