呼吸肌训练联合吞咽康复对头颈部肿瘤放射治疗患者的临床效果

Clinical Effect of Respiratory Muscle Training Combined with Swallowing Rehabilitation on Patients with Radiotherapy for Head-neck Tumors

目的:探究呼吸肌训练联合吞咽康复对头颈部肿瘤放射治疗患者的临床效果。方法:将2020年1月-2021年2月于九江市第一人民医院行放射治疗的50例头颈部肿瘤患者按照随机数字表法分为对照组(n=25)和观察组(n=25),对照组实施常规吞咽康复干预措施,观察组在对照组基础上实施呼吸肌训练干预方案。比较两组干预前及干预8周后吞咽功能[中文版安德森吞咽困难量表(MDADI)评分、功能性经口摄食量表(FOIS)评分]、吸气肌功能[最大吸气压(MIP)、吸气峰流速(PIF)]、运动能力[6分钟步行试验(6-MWT)]、生存质量[Spitzer生活质量指数(QL-Index)评分]。结果:干预8周后,两组MDADI评分、FOIS评分、MIP、PIF、6-MWT、QL-Index评分均较干预前显著升高,且观察组均显著高于对照组(P<0.05)。结论:呼吸肌训练联合吞咽康复对头颈部肿瘤放射治疗患者的临床效果显著,可有效改善其吞咽功能及吸气肌功能,亦有助于提高患者运动能力及生存质量,推荐临床应用。

Objective:To explore the clinical effect of respiratory muscle training combined with swallowing rehabilitation on patients with radiotherapy for head-neck tumors.Method:A total of 50 patients with head-neck tumors who underwent radiotherapy in Jiujiang No.1 People’s Hospital from January 2020 and February 2021 were divided into control group (n=25) and observation group (n=25) according to random number table method,the control group implemented conventional swallowing rehabilitation interventions,and the observation group implemented respiratory muscle training intervention regimen on the basis of the control group.The swallowing function[Chinese version of MD Anderson Dysphagia Inventory (MDADI) score,Functional Oral Ingestion Scale (FOIS) score],inspiratory muscle function[maximal inspiratory pressure (MIP),peak inspiratory flow (PIF)],exercise capacity[6-min walking test (6-MWT)]and quality of life[Spitzer Quality of Life Index (QL-Index) score]were compared between the two groups before intervention and after 8 weeks of intervention.Result:After 8 weeks of intervention,the scores of MDADI,scores of FOIS,MIP,PIF,6-MWT and scores of QL-Index of the two groups were significantly increased compared with those before intervention,and those of observation group were significantly higher than those of control group (P<0.05).Conclusion:Respiratory muscle training combined with swallowing rehabilitation has a significant clinical effect on patients with radiotherapy for head-neck tumors,and it can effectively improve the swallowing function and inspiratory muscle function,it also helps to improve the exercise capacity and quality of life,thus it is recommended for clinical application.
