香连丸联合美沙拉嗪治疗湿热蕴结型克罗恩病的临床研究

Clinical Study of Xianglian Pills Combined with Mesalazine in the Treatment of Damp-heat Accumulation Crohn’s Disease

目的:探讨香连丸联合美沙拉嗪对湿热蕴结型克罗恩病患者的临床疗效及血清中相关炎性因子水平的影响。方法:收集2019年10月-2021年9月就诊于湖南省中医药研究院附属医院的湿热蕴结型克罗恩病患者60例,按照随机数字表法将其分为对照组和观察组,每组30例。对照组给予美沙拉嗪治疗,观察组给予香连丸联合美沙拉嗪治疗,两组患者均连续治疗4周。比较两组治疗后临床总有效率,治疗前后Best的克罗恩病活动指数评分(Best CDAI)及炎症性肠病问卷(IBDQ)评分,比较两组血清中相关炎症因子水平的变化。结果:观察组的临床总有效率高于对照组,差异有统计学意义(P<0.05)。与治疗前相比,两组治疗后的Best CDAI评分均明显下降,IBDQ评分均显著升高(P<0.05)。与对照组相比,观察组治疗后Best CDAI评分明显降低,IBDQ评分明显升高(P<0.05)。与治疗前相比,两组治疗后血清中TNF-α、IL-6、IL-17水平均明显降低,IL-10水平均显著升高(P<0.05)。与对照组相比,观察组治疗后血清中TNF-α、IL-6、IL-17水平均明显降低,IL-10水平显著升高(P<0.05)。结论:香连丸联合美沙拉嗪能有效地抑制克罗恩病患者体内炎症状态,调控促炎与抗炎因子之间的动态平衡,缓解患者临床症状,提高其生存质量,其疗效明显优于单纯使用美沙拉嗪治疗。

Objective:To investigate the clinical effect of Xianglian Pills combined with Mesalazine on patients with damp-heat accumulation Crohn’s disease and the level of related inflammatory factors in serum.Method:A total of 60 patients with damp-heat accumulation type Crohn’s disease who were treated in the Affiliated Hospital of Hunan Institute of Traditional Chinese Medicine from October 2019 to September 2021 were collected,according to random number table method,they were divided into control group and observation group,30 cases in each group.The control group was treated with Mesalazine,and the observation group was treated with Xianglian Pills combined with Mesalazine,the patients in the two groups were treated for 4 weeks.The total clinical effective rate,Best Crohn’s disease activity index score(Best CDAI)and inflammatory bowel disease questionnaire(IBDQ)before and after treatment were compared,and the changes of serum inflammatory factors in the two groups were compared.Result:The total clinical effective rate of the observation group was higher than that of the control group,the difference was statistically significant(P<0.05).Compared with before treatment,the Best CDAI score were significantly decreased and the IBDQ score were significantly increased in both groups after treatment(P<0.05).Compared with the control group,the Best CDAI score in the observation group was significantly decreased and the IBDQ score increased significantly after treatment(P<0.05).Compared with before treatment,the levels of TNF-α,IL-6 and IL-17 in serum in two groups were significantly decreased after treatment,while the levels of IL-10 were significantly increased(P<0.05).Compared with the control group,the serum levels of TNF-α,IL-6 and IL-17 in observation group were significantly decreased after treatment,while the level of IL-10 was significantly increased(P<0.05).Conclusion:Xianglian Pills combined with Mesalazine can effectively inhibit the inflammatory state in patients with Crohn’s disease,regulate the dynamic balance between pro-inflammatory and anti-inflammatory factors,alleviate patients’clinical symptoms and improve their quality of life,its curative effect is significantly better than that of mesalazine alone.
