补血生乳汤结合中频电刺激穴位在产后早期乳汁分泌中的应用效果研究 被引量:5

Study on Application Effect of Busxue Shengru Decoction(补血生乳汤)Combined with Medium Frequency Electric Stimulation in Early Postpartum Milk Secretion

摘要 目的探讨补血生乳汤结合中频电刺激穴位在产后早期乳汁分泌中的应用效果。方法选择2019年1月—2019年5月医院收治的气血两虚型产妇390例,依据随机对照法将患者分为研究组和对照组,每组195例,对照组产妇在产后加强营养,指导正确哺乳,在分娩24 h后,每日早晚取少泽、膻中、乳根三穴进行电刺激,每次20 min。研究组在对照组治疗的基础上,口服补血生乳汤治疗。两组均治疗7 d。比较两组产后7 d乳房饱胀感、泌乳量,比较产后24 h、48 h、产后7 d血清催乳素水平,随访1个月,比较两组母乳喂养率。比较研究组中顺产组和剖宫产组产妇产后7 d乳房饱胀感、泌乳量,比较产后24 h、48 h、产后7 d血清催乳素水平。结果研究组产妇中乳房饱胀感为Ⅰ级所占的比重高于对照组,乳房充盈度为Ⅱ级、Ⅲ级的所占的比重低于对照组(P<0.05),研究组产妇7 d泌乳量中、多的产妇占的比重高于对照组,泌乳量少的产妇低于对照组(P<0.05);研究组产妇产后24 h和48 h血清催乳素水平高于对照组(P<0.05),两组产妇产后7 d血清催乳素水平比较差异无统计学意义(P>0.05);研究组母乳喂养率较对照组高(P<0.05);研究组中顺产组产妇3 d乳房饱胀感Ⅱ级、Ⅲ级比例高于剖宫产组产妇,Ⅰ级低于剖宫产组(P<0.05);顺产组产妇产后24 h、48 h血清催乳素水平高于剖宫产组(P<0.05),两组产妇产后7 d血清催乳素水平比较差异无统计学意义(P>0.05)。结论分娩方式是影响产后早期乳汁分泌的重要因素,补血生乳汤结合中频电刺激能在产后早期提升乳房饱胀感,促进早期乳汁分泌,提升血清催乳素水平,增强产妇母乳喂养信心,提高母乳喂养率。 Objective To explore the effect of Busxue Shengru Decoction(补血生乳汤)combined with medium frequency electric stimulation on early postpartum milk secretion.Methods A total of 390 parturients with deficiency of both Qi and blood treated in our hospital from January 2019 to may 2019 were selected.According to the random control method,the patients were divided into the study group(n=195)and the control group(n=195).The parturients in the control group strengthened nutrition and guided correct lactation after delivery.24 hours after delivery,Shaoze(SI1),Danzhong(RN17)and Rugen(ST18)were taken every morning and evening for electrical stimulation for 20 minutes each time.The study group was treated with Buxue Shengru Decoction on the basis of the control group.Both groups were treated for 7 days.The breast fullness and lactation of the two groups at 7 days postpartum were compared,and the serum prolactin levels at 24 hours,48 hours and 7 days postpartum were compared.The breast feeding rates of the two groups were followed up for 1 month.The breast fullness and lactation volume of parturients in the spontaneous delivery group and cesarean section group at 7 days postpartum were compared,and the serum prolactin levels at 24 hours,48 hours and 7 days postpartum were compared.Results The proportion of breast fullness grade I in the study group was higher than that in the control group,and the proportion of breast fullness grade II and III was lower than that in the control group(P<0.05).The proportion of middle and high lactation in the study group was higher than that in the control group,and the proportion of less lactation in the study group was lower than that in the control group(P<0.05).The level of serum prolactin in the study group was higher than that in the control group at 24 and 48 hours postpartum(P<0.05).There was no significant difference between the study group and the control group at 7 days postpartum(P>0.05).The breastfeeding rate in the study group was higher than that in the control group(P<0.05).In the study group,the proportion of grade Ⅱ and Ⅲ breast fullness in spontaneous delivery group was higher than that in cesarean section group,and grade Ⅰ was lower than that in cesarean section group(P<0.05).The level of serum prolactin at 24 h and 48 h postpartum in spontaneous labor group was higher than that in cesarean section group(P<0.05).There was no significant difference between the two groups at 7 days postpartum(P>0.05).Conclusion The way of delivery is an important factor affecting milk secretion.The combination of Busxue Shengru Decoction with medium frequency electric stimulation can improve breast fullness,promote early milk secretion,increase serum prolactin level and improve the rate of breast feeding,which is worthy of further clinical study.
