低频脉冲电刺激联合摄食训练对脑卒中后吞咽障碍康复的促进作用 被引量:4

Effect of low frequency pulse electrical stimulation combined with feeding training on rehabilitation of dysphagia after stroke

摘要 目的探讨低频脉冲电刺激联合摄食训练对脑卒中后吞咽障碍康复的促进作用。方法依据随机数字表法将2019年1月至2021年1月于郑州大学第五附属医院收治的70例脑卒中后吞咽障碍患者分为两组,每组35例。两组均给予常规治疗,在此基础上,对照组实施摄食训练,观察组实施低频脉冲电刺激联合摄食训练治疗,两组均持续治疗2个月。比较两组治疗前及治疗2个月的吞咽功能包括标准吞咽功能评估量表(SSA),生活质量包括吞咽障碍特异性生活质量量表(SWAL-QOF)及舌骨喉复合体动度。结果与治疗前比较,治疗2个月两组临床检查、吞咽5 ml、吞咽60 ml评分降低,治疗2个月两组生活质量、吞咽症状评分、舌骨向前、舌骨上移、喉向前及喉上移最大幅度均增加,且与对照组比较,观察组临床检查、吞咽5 ml、吞咽60 ml评分降低,生活质量、吞咽症状评分、舌骨向前、舌骨上移、喉向前及喉上移最大幅度均增加(P<0.05)。结论低频脉冲电刺激联合摄食训练可改善脑卒中患者吞咽功能,促进舌骨喉复合体动度恢复,提高其生活质量。 Objective To explore the effect of low frequency pulse electrical stimulation combined with feeding training on the rehabilitation of dysphagia after stroke.Methods Seventy patients with dysphagia after stroke who were treated in the Fifth Affiliated Hospital of Zhengzhou University from January 2019 to January 2021 were randomly divided into two groups,35 cases in each group.Both groups were given conventional treatment.On this basis,the control group was given feeding training,and the observation group was given low-frequency pulse electrical stimulation combined with feeding training.Both groups were treated for 2 months.The swallowing function(SSA),quality of life swallowing disorder specific quality of life(SWAL-QOF)and hyoid laryngeal complex activity were compared between the two groups before and 2 months after treatment.Results Compared with before treatment,the scores of clinical examination,swallowing 5 ml and swallowing 60 ml in the two groups decreased significantly after 2 months of treatment.The scores of quality of life,swallowing symptoms,hyoid bone forward,hyoid bone upward,larynx forward and larynx upward in the two groups increased significantly after 2 months of treatment.Compared with the control group,the scores of clinical examination,swallowing 5 ml and swallowing 60 ml in the observation group decreased significantly,and the scores of quality of life,swallowing symptoms,hyoid bone forward,hyoid bone upward,larynx forward and larynx upward increased significantly.Swallowing symptom score,hyoid forward,hyoid upward,laryngeal forward and laryngeal upward were significantly increased(P<0.05).Conclusions Low frequency pulse electrical stimulation combined with feeding training can improve the swallowing function of stroke patients,promote the recovery of hyoid laryngeal complex and improve their quality of life.
