双源CT双能肺灌注成像对肺栓塞的诊断价值 被引量:3

Diagnostic value of dual energy lung perfusion imaging using dual source CT for pulmonary embolism

摘要 目的探讨双源CT双能肺灌注成像对肺栓塞的诊断价值。方法回顾性纳入2019年1月至2020年12月于河南省胸科医院行双源CT检查的疑似肺栓塞患者86例,对其图像进行处理和分析,得到电脑断层扫描血管造影(CTA)图像和肺灌注血容量(PBV)图像。以CTA图像作为对照,统计PBV图像诊断肺栓塞的敏感性、特异性、阳性和阴性预测值。结果PBV图像诊断肺栓塞的敏感性为94.12%(48/51),特异性为60.00%(21/35),阳性预测值为77.42%(48/62),阴性预测值为87.50%(21/24),PBV图像诊断完全肺栓塞的敏感性和特异性均为100%(16/16)。结论双源CT双能肺灌注成像能获得肺部、肺部动脉的解剖学信息和肺部血流灌注情况,据此可获得较多诊断肺栓塞的有效信息。 Objective To investigate the diagnostic value of dual energy lung perfusion imaging using dual source CT for pulmonary embolism.Methods A retrospectively enrolled 86 patients with suspected pulmonary embolism who underwent dual source CT examinations in Henan Provincaal Chest Hospital from January 2019 to December 2020,and their images were processed and analyzed,obtained CT angiography(CTA)images and pulmonary perfusion blood volume(PBV)images.CTA images were used as a control,and the sensitivity,specificity,positive and negative predictive values of PBV images in the diagnosis of pulmonary embolism were calculated.Results The sensitivity of PBV images in diagnosing pulmonary embolism was 94.12%(48/51),the specificity was 60.00%(21/35),the positive predictive value was 77.42%(48/62),and the negative predictive value was 87.50%(21/24),the sensitivity and specificity of PBV images in the diagnosis of complete pulmonary embolism are both 100%(16/16).Conclusions Dual energy lung perfusion imaging using dual source CT can obtain the anatomical information of the lungs and pulmonary arteries and the pulmonary blood perfusion,more effective information for the diagnosis of pulmonary embolism can be obtained.
