青年2型糖尿病患者血清25羟维生素D;水平与代谢综合征的关系 被引量:11

Relationship between serum 25-hydroxyvitamin D3 level and the risk of metabolic syndrome in young patients with type 2 diabetes mellitus

摘要 目的探讨青年2型糖尿病患者血清25羟维生素D3[25(OH)D3]水平与代谢综合征(MS)患病风险的关系。方法选取16~44岁的448例2型糖尿病患者,根据是否合并MS分为对照(CON)组(159例)和MS组(289例),比较两组患者生化代谢指标水平以及血清25(OH)D3缺乏情况,分析血清25(OH)D3水平与MS的相关性。结果MS组血清25(OH)D3水平明显低于CON组(P<0.05)。与CON组比较,MS组吸烟史明显更长(P<0.05),腰围、腰臀比、体重指数、空腹血糖、收缩压、舒张压、甘油三酯明显增高,高密度脂蛋白-胆固醇(HDL-C)和25(OH)D3水平明显降低(P均<0.05)。不同代谢性疾病中血清25(OH)D3缺乏情况比较,各组腹型肥胖、高血压、高甘油三酯、低HDL-C患者数,均有显著差异,血清25(OH)D3缺乏>不足>充足(P均<0.05),血清25(OH)D3水平与吸烟、腰围、腰臀比、体重指数、空腹血糖、糖化血红蛋白A1c(HbA1c)、收缩压、舒张压、甘油三酯、LDL-C呈负相关,与HDL-C呈正相关,多元logistic回归分析显示血清25(OH)D3充足是T2DM合并MS的独立保护因素。结论青年T2DM患者血清25(OH)D3水平与MS患病风险呈独立负相关。 Objective Investigate the relationship between serum 25-hydroxyvitamin D3[25(OH)D3]level and the risk of metabolic syndrome(MS)in young patients with type 2 diabetes.Methods A total of 448 type 2 diabetes mellitus patients aged 16 to 44 years were selected as the study subjects.The patients were divided into the control(CON)group(159 cases)and the MS group(289 cases)based on whether the patients had MetS.The levels of biochemical and metabolic indicators and the lack of serum 25(OH)D3 were compared between the two groups,and the correlation between serum 25(OH)D3 levels and MS was analyzed.Results The serum 25(OH)D3 level in MS group was significantly lower than that in CON group(P<0.05).Compared with CON group,the smoking history of the MS group was significantly longer(P<0.05),waist circumference,waist-hip ratio,body mass index(BMI),fasting blood glucose,systolic blood pressure,diastolic blood pressure,triglyceride were significantly increased,and high density lipoprotein cholesterol(HDL-C)and 25(OH)D3 levels were significantly reduced(all P<0.05).The number of MS components decreased in the order of serum 25(OH)D3 deficiency>insufficient>sufficient(all P<0.05).The serum 25(OH)D3 level was related to smoking,waist circumference,waist-hip ratio,and BMI,FBG,hemoglobinA1c(HbA1c),systolic blood pressure,diastolic blood pressure,triglyceride,and low density lipoprotein cholesterol(LDL-C)were negatively correlated and positively correlated with HDL-C.Multiple logistic regression analysis showd that sufficient serum 25(OH)D3 was an independent protective factor for T2DM with MS.Conclusion In young patients with type 2 diabetes,serum 25(OH)D3 levels are independently and negatively related to the risk of MS.
