NCPAP和NIPPV分别联合肺表面活性物质治疗新生儿呼吸窘迫综合征的效果 被引量:1

Effect of NCPAP and NIPPV Respectively Combined with Pulmonary Surfactant in the Treatment of Neonatal Respiratory Distress Syndrome

摘要 目的:比较经鼻持续正压通气(nasal continuous positive airway pressure,NCPAP)和经鼻间歇正压通气(nasal intermittent positive pressure ventilation,NIPPV)分别联合肺表面活性物质(pulmonary surfactant,PS)治疗新生儿呼吸窘迫综合征(neonatal respiratory distress syndrome,NRDS)的效果。方法:选取广宁县人民医院2018年3月-2020年5月70例NRDS患儿。按照计算机分组法将其分为对照组(34例)和观察组(36例)。两组均给予基础治疗措施,对照组给予NCPAP联合PS治疗,观察组给予NIPPV联合PS治疗。观察两组临床疗效,以及治疗前及治疗后3 d血气指标水平及血清细胞因子水平。结果:治疗后,观察组总有效率(91.67%)高于对照组(73.53%)(P<0.05);治疗后3 d,观察组血氧饱和度(SaO_(2))、氧合指数(OI)、血氧分压(PaO_(2))水平均高于对照组(P<0.05);治疗后3 d,观察组巨噬细胞移动抑制因子-1(MIF-1)、高迁移率族蛋白1(HMGB-1)、Ⅱ型肺泡细胞表面抗原(KL-6)水平均低于对照组(P<0.05)。结论:与NCPAP比较,NIPPV联合PS治疗NRDS能显著改善血气指标,缓解肺泡损伤及炎症反应,进而提高治疗效果。 Objective:To compare the effects of nasal continuous positive airway pressure (NCPAP) ventilation and nasal intermittent positive pressure ventilation (NIPPV) respectively combined with pulmonary surfactant (PS) in the treatment of neonatal respiratory distress syndrome (NRDS).Method:Seventy children with NRDS in Guangning People’s Hospital from March 2018 to May 2020 were selected.They were divided into control group (34 cases) and observation group (36 cases) according to computer grouping method.Both groups were given basic treatment measures,the control group was given the NCPAP combined with PS treatment,and the observation group was given the NIPPV combined with PS treatment.The clinical efficacy,blood gas index levels and serum cytokine levels before and 3 d after treatment were observed.Result:After treatment,the total effective rate of the observation group (91.67%) was higher than that of the control group (73.53%) (P<0.05);3 d after treatment,the levels of blood oxygen saturation (SaO_(2)),oxygenation index (OI) and blood partial pressure of oxygen (PaO_(2)) in observation group were higher than those in control group (P<0.05);3 d after treatment,the levels of macrophage migration inhibitory factor-1 (MIF-1),high mobility group protein-1 (HMGB-1) and type Ⅱ alveolar cell surface antigen (KL-6) in the observation group were lower than those in the control group (P<0.05).Conclusion:Compared with NCPAP,NIPPV combined with PS in the treatment of NRDS can significantly improve blood gas index,alleviate alveolar injury and inflammatory reaction,and then improve the therapeutic effect.
