免疫增强型肠内营养乳剂对COPD机械通气患者营养状态和预后的影响

Effects of Immune-enhanced Enteral Nutritional Emulsion on Nutritional Status and Prognosis of Patients with COPD with Mechanical Ventilation

目的:分析免疫增强型肠内营养乳剂对慢性阻塞性肺病(COPD)机械通气患者营养状态和预后的影响,以期为COPD机械通气患者有效恢复奠定理论基础。方法:以2018年3月-2021年1月来肇庆市高要区中医院治疗的82例COPD机械通气患者为研究对象,随机分为对照组和观察组,各41例。对照组采用常规治疗及常规肠内营养乳剂,观察组在常规治疗基础上使用免疫增强型肠内营养乳剂。比较两组免疫功能、营养指标及并发症发生情况。结果:治疗前,两组IgA、IgG、淋巴细胞计数(LYM)、血清白蛋白(ALB)、前白蛋白(PA)、转铁蛋白(TRF)、血红蛋白(Hb)水平差异均无统计学意义(P>0.05);治疗14 d后,观察组IgA、IgG、LYM、ALB、PA、TRF、Hb水平和对照组IgA、IgG、ALB、PA水平均有显著升高,与治疗前比较差异有统计学意义(P<0.05),且观察组IgA、IgG、LYM、ALB、PA、TRF、Hb水平的改善幅度大于对照组(P<0.05)。观察组并发症发生率低于对照组(P<0.05)。结论:免疫增强型肠内营养乳剂可以有效提高COPD机械通气患者的免疫功能,同时改善营养状态,降低并发症发生率,可以促进良好的预后。

Objective:To analyze the effect of immune-enhanced enteral nutrition emulsion on the nutritional status and prognosis of patients with Chronic obstructive pulmonary disease (COPD) following mechanical ventilation,so as to lay a theoretical foundation for the effective recovery of COPD patients with mechanical ventilation.Method:A total of 82 patients with COPD patients following mechanical ventilation who came to Zhaoqing Gaoyao District Hospital of Traditional Chinese Medicine for treatment from March 2018 to January 2021 were randomly divided into control group and observation group,with 41 patients in each group.The control group received routine postoperative treatment and routine enteral nutrition,while the observation group received immune-enhanced enteral nutrition emulsion on the basis of routine treatment.The immune function,nutritional indicators and complications were compared between the two groups.Result:Before treatment,there were no significant differences in IgA,IgG,LYM ALB,PA,TRF and Hb levels between the two groups (P>0.05).After 14 days of treatment,the levels of IgA,IgG,LYM,ALB,PA,TRF and Hb in the observation group were significantly improved,the levels of IgA,IgG,ALB and PA in the control group were significantly improved,the improvement of IgA,IgG,LYM,ALB,PA,TRF and Hb levels in the observation group were significantly better than those in the control group (P<0.05).The incidence of complications in the observation group was significantly lower than that in the control group (P<0.05).Conclusion:Immune-enhanced enteral nutrition can effectively improve the immune function of patients with COPD with mechanical ventilation,improve nutritional status,reduce the incidence of complications,promote good prognosis.
