高血压合并代谢综合征以及单纯高血压患者血清胱抑素C水平与血管内皮功能的相关性 被引量:2

Correlation analysis of serum cystatin C and vascular endothelial function in hypertensive patients with metabolic syndrome and simple hypertension

摘要 目的:探讨高血压合并代谢综合征(MS)以及单纯高血压患者血清胱抑素C与血管内皮功能的相关性。方法:选取2017年1月~2018年6月收治的高血压合并MS患者280例作为高血压合并MS组,再选择同期单纯高血压患者220例作为单纯高血压组。比较两组患者一般资料、血清学指标和血管内皮功能的差异,分析血流介导的舒张功能(FMD)与各指标水平的相关性以及FMD的影响因素。结果:①高血压合并MS组体重指数(BMI)、舒张压、空腹血糖(FPG)、总胆固醇(TC)、三酰甘油(TG)、低密度脂蛋白胆固醇(LDL-C)、尿酸、肌酐、尿素氮和胱抑素C水平均显著高于单纯高血压组,高密度脂蛋白胆固醇(HDL-C)和FMD水平显著低于单纯高血压组,差异有统计学意义(P<0.01)。②两组胱抑素C水平均随FMD的上升而呈下降趋势,差异有统计学意义(P<0.01)。③FMD与高血压合并MS患者的年龄、FPG、TG、尿酸、胱抑素C水平呈负相关,差异有统计学意义(r=-0.897、-0.598、-0.382、-0.399、-0.527,P<0.01);FMD与单纯高血压患者年龄、收缩压、胱抑素C水平呈负相关,差异有统计学意义(r=-0.947、-0.311、-0.546,P<0.01)。④高血压合并MS组中,年龄、FPG、TG、尿酸和胱抑素C均是FMD的影响因素,而单纯高血压组中,年龄、收缩压和胱抑素C是FMD的影响因素。结论:高血压合并MS患者以及单纯高血压患者血清胱抑素C水平均与血管内皮功能呈负相关。 Objective To explore the correlation analysis of serum cystatin C and vascular endothelial function in hypertensive patients with metabolic syndrome(MS)and simple hypertension.Method A total of 280 patients with hypertension complicated and MS admitted to our hospital from January 2017 to June 2018 were selected as the hypertension+MS group.Another 220 patients with simple hypertension who were treated in the same period in our hospital were selected as the simple hypertension group.The differences in general information,serological indexes and vascular endothelial function between the two groups were compared,and the correlation between the levels of FMD and each index was analyzed,as well as the influencing factors of FMD.Results①The levels of body mass index(BMI),diastolic pressure,fasting plasma glucose(FPG),total cholesterol(TC),triglyceride(TG),low density lipoprotein cholesterol(LDL-C),uric acid,creatinine,urea nitrogen,cystatin C in hypertension+MS group were significantly higher than in simple hypertension group,while the levels of high-density lipoprotein cholesterol(HDL-C)and blood flow mediated endothelial dependent vasodilation function(FMD)were significantly lower than in simple hypertension group,the difference was statistically significant(P<0.01).②Cystatin C level of subjects in both groups showed a decreasing trend with the increase of FMD,and the difference was statistically significant(P<0.01).③FMD was negatively correlated with age,FPG,TG,uric acid,cystatin C of hypertension+MS subjects(r=-0.897,-0.598,-0.382,-0.399,-0.527,P<0.01).FMD was negatively correlated with age,systolic pressure and cystatin C of simple hypertension subjects(r=-0.947,-0.311,-0.546,P<0.01).④Age,FPG,TG,uric acid and cystatin C were the influencing factors of FMD in the hypertensive MS group,while age,systolic blood pressure and cystatin C were the influencing factors of FMD in the simple hypertension group.Conclusion Serum cystatin C levels were negatively correlated with vascular endothelial function in hypertensive patients with MS and patients with simple hypertension.
