发散式体外冲击波对急性期钙化性冈上肌腱炎的疗效

Efficacy of Divergent Extracorporeal Shock Wave on Calcified Supraspinatus Tendinitis of Acute Stage

目的:探讨发散式体外冲击波对急性期钙化性冈上肌腱炎的疗效。方法:选取2018年1月-2019年12月于青州市中医院和潍坊市市直机关医院收治的70例急性期钙化性冈上肌腱炎患者,采用随机数字表法分为治疗组35例和对照组35例,对照组给予局部类固醇类药物注射治疗,治疗组在此基础上加用发散式体外冲击波治疗。分别于治疗前、治疗后1、6、12个月比较两组的数字评价量表(NRS)、美国加州大学肩关节评分系统(UCLA)、Constant-Murley肩关节功能评分和钙化灶的吸收率。结果:治疗前,两组NRS、UCLA和Constant-Murley评分比较,差异均无统计学意义(P>0.05)。两组治疗后1、6、12个月的NRS、UCLA和Constant-Murley评分均较治疗前明显改善(P<0.05),但治疗后1个月,两组NRS、UCLA和Constant-Murley评分比较,差异均无统计学意义(P>0.05)。治疗后6、12个月,治疗组NRS、UCLA和Constant-Murley评分均明显优于对照组(P<0.05)。治疗后1个月,两组钙化灶吸收率比较,差异无统计学意义(P>0.05);治疗后6、12个月,治疗组钙化灶吸收率均明显优于对照组(P<0.05)。结论:发散式体外冲击波联合局部类固醇类药物注射治疗,能减轻患者疼痛,促进肩关节功能恢复,促进钙化灶吸收,改变了单一应用类固醇类激素远期疗效欠佳的问题,是治疗急性期钙化性冈上肌腱炎的有效方法。

Objective:To investigate the efficacy of divergent extracorporeal shock wave on calcified supraspinatus tendinitis of acute stage.Method:A total of 70 patients with calcified supraspinatus tendinitis of acute stage treated in Qingzhou Hospital of Traditional Chinese Medicine and Weifang Shizhi Jiguan Hospital from January 2018 to December 2019 were divided into 35 cases in the treatment group and 35 cases in the control group by random number table method.The control group was treated with local steroid injection,and the treatment group was treated with divergent extracorporeal shock wave on the basis of the control group.Numerical rating scale(NRS),the University of California at Los Angeles shoulder rating scale(UCLA)and Constant-Murley symptom score and the reduction rate of calcification were compared between the two groups before treatment and 1,6,12 months after treatment.Result:Before treatment,there were no significant differences in NRS,UCLA and Constant-Murley scores between the two groups(P>0.05).The NRS,UCLA and Constant-Murley scores of the two groups at 1,6 and 12 months after treatment were significantly improved compared with those before treatment(P<0.05),but there were no significant differences between the two groups at 1 month after treatment(P>0.05).At 6 and 12 months after treatment,the NRS,UCLA and Constant-Murley scores of the treatment group were significantly better than those of the control group(P<0.05).1 month after treatment,there was no significant difference in the reduction rate of calcification between the two groups(P>0.05);6 and 12 months after treatment,the reduction rates of calcification in the treatment group were significantly better than those in the control group(P<0.05).Conclusion:Divergent extracorporeal shock wave combined with steroid hormone can relieve pain,improve the function of shoulder joint,reduce the calcification,solve the question of poor long-term result of applying with steroid hormone,it is an effective treatment for calcified supraspinatus tendinitis of acute stage.
