腹腔镜下小切口手术治疗胃十二指肠溃疡穿孔的效果及并发症发生率研究

Efficacy and Complication Rate of Laparoscopic Small Incision Surgery in the Treatment of Perforation of Gastroduodenal Ulcer

目的:探讨腹腔镜下小切口手术治疗胃十二指肠溃疡穿孔的效果及并发症发生率,为后续治疗奠定基础。方法:回顾性选择河源市人民医院2019年1月-2020年12月收治的128例进行胃十二指肠溃疡穿孔治疗的患者为研究对象,按照手术方式的不同分为观察组和对照组,其中81例进行腹腔镜手术患者为观察组,47例常规手术患者为对照组。比较两组治疗效果及并发症发生情况。结果:观察组术中出血量少于对照组,切口长度及住院时间显著短于对照组,差异均有统计学意义(P<0.05);观察组肛门首次排气时间、胃肠减压时间显著优于对照组,差异均有统计学意义(P<0.05);观察组并发症发生率为1.23%,低于对照组的12.77%,差异有统计学意义(P<0.05)。结论:腹腔镜下小切口手术治疗胃十二指肠溃疡穿孔不但可以有效提升效果,同时可减少并发症发生,优势明显,可在临床中推广应用。

Objective:To explore the efficacy and complication rate of laparoscopic small incision surgery in the treatment of perforation of gastroduodenal ulcer,so as to lay a foundation for follow-up treatment.Method:A total of 128 patients with gastroduodenal ulcer perforation treated in Heyuan People’s Hospital from January 2019 to December 2020 were retrospectively selected as the research object.According to different surgical methods,they were divided into observation group and control group,81 patients underwent laparoscopic surgery as the observation group and 47 patients underwent routine surgery as the control group.The therapeutic effects and complications of the two groups were observed and compared.Result:The intraoperative blood loss was less than that in control group,incision length and hospital stay in observation group were significantly shorter than those in control group,and the differences were statistically significant(P<0.05);the first anal exhaust time and gastrointestinal decompression time in observation group were better than those in control group,and the differences were statistically significant(P<0.05).The incidence of complications in observation group(1.23%)was lower than that in control group(12.77%),and the difference was statistically significant(P<0.05).Conclusion:Laparoscopic small incision surgery for perforation of gastroduodenal ulcer can not only effectively improve the curative effect,but also reduce the the complications,which has obvious advantages and can be widely used in clinic.
