BDNF及其受体TrkB在癌症合并创伤后应激障碍患者外周血中的表达及意义

Expression and Significance of BDNF and Its Receptor TrkB in Peripheral Blood of Cancer Patients with Post-traumatic Stress Disorder

摘要 目的:观察脑源性神经营养因子(BDNF)及其受体酪氨酸激酶B(TrkB)在癌症合并创伤后应激障碍(PTSD)患者外周血中的表达,并探讨其临床意义。方法:选取2018年10月-2020年1月鄂州市中心医院葛山分院收治的57例癌症合并PTSD患者作为观察组,同期选择55例癌症患者作为对照组。采用Western blot法检测BDNF及其受体TrkB在两组患者外周血中的表达水平,并采用汉密尔顿抑郁(HAMD)及汉密尔顿焦虑(HAMA)量表评估两组患者抑郁及焦虑程度,分析BDNF及其受体TrkB蛋白表达与HAMD、HAMA量表评分的相关性关系。结果:观察组外周血BDNF及其受体TrkB蛋白表达水平均低于对照组,差异均有统计学意义(P<0.05)。观察组绝望感、睡眠障碍、全身症状、阻滞、认识障碍、焦虑/躯体化及HAMD总分均高于对照组,差异均有统计学意义(P<0.05)。观察组精神性焦虑、躯体性焦虑及HAMA总分均高于对照组,差异均有统计学意义(P<0.05)。BDNF及其受体TrkB蛋白表达与HAMD、HAMA量表评分均呈负相关(P<0.05)。结论:癌症合并PTSD患者外周血BDNF及其受体TrkB蛋白表达水平明显下降,在诊断癌症患者是否合并PTSD和评估抑郁、焦虑症状严重程度方面均具有一定价值。 Objective:To observe the expression of brain-derived neurotrophic factor (BDNF) and its receptor tyrosine kinase B (TrkB) in peripheral blood of cancer patients with post-traumatic stress disorder (PTSD),and to explore its clinical significance.Method:A total of 57 cancer patients with PTSD admitted to Geshan Branch of Central Hospital of Ezhou from October 2018 to January 2020 were selected as the observation group,and 55 cancer patients were selected as the control group during the same period.Western blot was used to detect the expression levels of BDNF and its receptor TrkB in peripheral blood of patients in the two groups,and Hamilton depression scale (HAMD) and Hamilton anxiety scale (HAMA) scales were used to evaluate the degree of depression and anxiety of patients in the two groups,and the correlation between the protein expression of BDNF and its receptor TrkB and HAMD and HAMA scale scores was analyzed.Result:The expression levels of BDNF and its receptor TrkB protein in peripheral blood in the observation group were lower than those in the control group,the differences were statistically significant (P<0.05).The total scores of despair,sleep disturbance,systemic symptoms,block,cognitive impairment,anxiety/somatization and total HAMD scores in the observation group were higher than those in the control group,the differences were statistically significant (P<0.05).The mental anxiety,somatic anxiety and total HAMA scores in the observation group were higher than those in the control group,and the differences were statistically significant (P<0.05).The expression of BDNF and its receptor TrkB were negatively correlated with HAMD and HAMA scale scores (P<0.05).Conclusion:BDNF and its receptor TrkB protein expression levels in peripheral blood of cancer patients complicated with PTSD significantly decrease,which is of certain value in the diagnosis of patients with cancer complicated with PTSD and assessment of the severity of depression and anxiety symptoms.
