摘要 目的:探讨自体富血小板凝胶(PRP)对难愈性创面患者的影响。方法:选取2019年9月-2020年12月重庆医科大学附属第二医院急救部收治的122例难愈性创面患者,根据随机数字表法分为对照组(n=61)和研究组(n=61),对照组实施单纯皮瓣移植,研究组实施PRP辅助皮瓣移植,比较两组治疗有效率,治疗前后炎性因子、生长因子水平,并发症发生率。结果:研究组治疗有效率高于对照组(P<0.05);治疗后,研究组创面肉芽组织γ-干扰素(INF-γ)、白细胞介素-6(IL-6)、C反应蛋白(CRP)水平均低于对照组(P<0.05);研究组创面肉芽组织血管内皮生长因子(VEGF)、结缔组织生长因子(CTGF)、转化生长因子-β_(1)(TGF-β_(1))水平均高于对照组(P<0.05);研究组并发症发生率低于对照组(P<0.05)。结论:针对难愈性创面患者,在皮瓣移植前予以PRP外敷可减轻局部炎症反应,促进创面生长,提高疗效,减少并发症的发生。 Objective:To investigate the effects of autologous platelet-rich gel(PRP)on patients with refractory wounds.Method:A total of 122 cases of refractory wound patients admitted to the Emergency Department,the Second Affiliated Hospital of Chongqing Medical University from September 2019 to December 2020 were selected,they were divided into the control group(n=61)and study group(n=61)according to the random number table method.The control group was treated with simple skin flap transplantation,and the study group was treated with PRP-assisted skin flap transplantation.The treatment effective rate,the levels of inflammatory factors and growth factors before and after treatment,and the incidence of complications were compared between groups.Result:The treatment effective rate in the study group was higher than that in the control group(P<0.05);after treatment,the levels of interferon-γ(INF-γ),interleukin-6(IL-6)and C-reactive protein(CRP)in the wound granulation tissue of the study group were lower than those of the control group(P<0.05);the levels of vascular endothelial growth factor(VEGF),connective tissue growth factor(CTGF)and transforming growth factor-β_(1)(TGF-β_(1))in wound granulation tissue in study group were higher than those in control group(P<0.05);the incidence of complication in the study group was lower than that in the control group(P<0.05).Conclusion:For patients with refractory wound,external application of PRP before skin flap transplantation can reduce local inflammatory reaction,promote the growth of wounds,improve the efficacy and reduce the incidence of complications.
