中国老年人慢性病共病对其健康状况的影响

Influence of multiple chronic conditions on the health status of the elderly in China

目的:了解中国老年人慢性病共病(MCCs)的患病情况,分析老年人慢性病共病对其健康状况的影响。方法:利用中国健康与养老追踪调查2018年随访数据,选取7 062名60岁以上老年人的社会人口学资料、慢性病患病情况及健康状况等信息。采用最小二乘法和概率模型probit两种估计方法分析慢性病共病对其生理和心理健康的影响。结果:共收集男3 125人(44.3%)、女3 937人(55.7%)的资料。老年人慢性病共病患病率为17.4%(1 232/7 062)。高血压+血脂异常是最常见二元疾病组合(16.2%,200/1 232),高血压+血脂异常+心脏病是最常见三元疾病组合(4.3%,53/1 232)。老年人慢性病共病对其日常生活活动和抑郁有显著正向影响。结论:我国老年人慢性病共病的患病率较高,社区医务工作者应加强老年人慢性病共病的防治与管理,以提高老年人的身心健康和生活质量。

Objective: To understand the prevalence of multiple chronic conditions and analyze the impact of multiple chronic conditions on the health status of the elderly in China. Methods: We enrolled 7 062 cases aged 60 or over from the participants of China Health and Retirement Longitudinal Study 2018 and collected their data of sociodemographics, chronic disease status and health status, and then we analyzed the effects of multiple chronic conditions on their physical and mental health by ordinary least squares and probit. Results: There were 3 125 males(44. 3%) and 3 937 females(55. 7%) included.About 17. 4%(1 232/7 062) of the participants had comorbidities. Hypertension plus dyslipidemia was the most common binary combination(16. 2%, 200/1 232), and hypertension plus dyslipidemia plus heart disease was the most common ternary combination(4. 3%, 53/1 232). Multiple chronic conditions had a significant positive effect on activities of daily living and depression in the elderly. Conclusion: The prevalence of multiple chronic conditions was high among the elderly in China, the community medical workers should strengthen the prevention and management of multiple chronic conditions among the elderly in order to improve their physical and mental health and life quality.
