甲氨蝶呤不同给药方案治疗异位妊娠患者的回顾性分析

Retrospective analysis on methotrexate in the treatment of ectopic pregnancy

目的了解本院甲氨蝶呤治疗异位妊娠患者的用药情况,为合理用药提供参考。方法回顾性分析2017年1月-2019年6月本院收治的甲氨蝶呤治疗异位妊娠患者199例,对用药前后β-h CG变化及血常规、肝肾功等不良反应发生情况进行统计,综合分析我院甲氨蝶呤治疗异位妊娠患者的合理性、有效性以及不良反应发生率。结果甲氨蝶呤单剂量方案(1mg/kg,D1)和多剂量方案(0.4 mg/kg/d,D1~D5)用药剂量差异不大,2种给药方案有效率比较,差异无统计学意义(P>0.05),但多剂量方案不良反应发生率较单剂量方案高。结论 2种治疗方案对异位妊娠患者临床疗效均较好且无差异,但甲氨蝶呤单剂量方案不良反应少,应用方便,安全性高,有药物保守治疗适应症的患者,此疗法可作为首选。

Objective To understand the use of methotrexate in the treatment of ectopic pregnancy in our hospital so as to provide reference for rational use of methotrexate. Methods 199 cases of ectopic pregnancy treated with methotrexate from Jan. 2017 to Jun.2019 in Yan’an hospital were analyzed retrospectively. It conducted statistic the changes of β-h CG and the adverse reactions before and after drug use. Comprehensively analyzed the incidence rate of rationality,effectiveness and adverse reactions of methotrexate in the treatment of ectopic pregnancy in our hospital. Results There was no significant difference in single dosage of methotrexate( 1 mg/kg,D1) and multidoses( 0. 4 mg/kg/d,D1 ~ D5),there was no statistic significance in the effective rate. However,the incidence of adverse reactions of multi dose regimen was higher than that of single dose regimen. Conclusions There is no difference and with better clinical effects between the two treatment schemes in the treatment of ectopic pregnancy. However,the single dose methotrexate scheme has fewer adverse reactions,with convenient use,high safety,and can be used as the first choice for patients with indications of conservative treatment.
