自拟补肾培元方对脾肾两虚型慢性肾功能衰竭患者的影响

Effect of Self-made Bushen Peiyuan Prescription on Patients with Chronic Renal Failure of Spleen-kidney Deficiency Type

目的:探讨自拟补肾培元方对脾肾两虚型慢性肾功能衰竭患者的影响。方法:选取2018年2月-2020年5月当阳市中医医院收治的120例慢性肾功能衰竭患者为研究对象。按照随机数表法将患者分为对照组(n=55)和观察组(n=65)。对照组给予常规治疗,观察组在上述基础上给予自拟补肾培元方治疗,治疗1个月后,比较两组临床疗效及治疗前后肾功能指标、血清肾纤维化指标。结果:观察组总有效率(93.85%)显著高于对照组(74.55%),差异有统计学意义(P<0.05);治疗后,两组肾小球滤过率估算值(eGFR)较治疗前升高,尿素氮(BUN)、血肌酐(Scr)水平较治疗前均降低,且观察组eGFR高于对照组,BUN、Scr水平均低于对照组(P<0.05);治疗后,两组层粘连蛋白(LN)、Ⅲ型前胶原蛋白(PC-Ⅲ)、血清透明质酸(HA)水平较治疗前均下降,观察组LN、PC-Ⅲ、HA水平低于对照组(P<0.05)。结论:自拟补肾培元方治疗脾肾两虚型慢性肾功能衰竭疗效明确,可改善肾功能,延缓患者肾纤维化。

Objective:To investigate the effect of self-made Bushen Peiyuan Prescription on patients with chronic renal failure of spleen-kidney deficiency type.Method:A total of 120 patients with chronic renal failure of spleen-kidney deficiency type admitted to Dangyang Hospital of Traditional Chinese Medicine from February 2018 to May 2020 were selected as the research objects.Patients were divided into control group (n=55) and observation group (n=65) according to random number table method.The control group was given conventional treatment,and the observation group was given self-made Bushen Peiyuan Prescription treatment on the above basis.After 1 month of treatment,the clinical efficacy,renal function indexes and serum renal fibrosis indexes before and after treatment of the two groups were compared.Result:The total effective rate of the observation group (93.85%) was significantly higher than that of the control group (74.55%),and the difference was statistically significant (P<0.05).After treatment,the estimated glomerular filtration rate (eGFR) was increased,the levels of blood urea nitrogen (BUN) and serum creatinine (Scr) were decreased,and eGFR in observation group was higher than that in control group,while BUN and Scr levels in observation group were lower than those in control group (P<0.05).After treatment,the levels of laminin (LN),procollagen type Ⅲ (PC-Ⅲ) and serum hyaluronic acid (HA) in both groups were decreased compared with those before treatment,and the levels of LN,PC-Ⅲ and HA in observation group were lower than those in control group (P<0.05).Conclusion:Self-made Bushen Peiyuan Prescription has definite curative effect on chronic renal failure of spleen-kidney deficiency type,which can improve renal function and delay renal fibrosis of patients.
