胰腺囊腺瘤MSCT影像学表现及鉴别诊断价值

MSCT Imaging Manifestations of Pancreatic Cystadenoma and Its Differential Diagnosis Value

目的分析胰腺囊腺瘤MSCT影像学表现及鉴别诊断价值。方法肿瘤大小、形态、密度及强化程度等特征及MSCT检查鉴别诊断胰腺囊腺瘤的准确率。结果32例胰腺囊腺瘤患者,经MSCT检查诊断准确率为90.63%。18例胰腺浆液性囊腺瘤共7例位于胰颈部及头颈部,5例位于胰体,6例位于胰腺尾部。CT扫描显示7例呈“蜂窝状”,边界清楚。3例CT表现纤维性中央瘢痕,伴特征性钙化,增强扫描后,肿瘤的蜂窝状结构更清晰。11例中心无瘢痕,囊内可见壁结节。CT表现单房囊性低密度影,边缘呈分叶状,内密度均匀。增强扫描无明显强化。胰腺粘液性囊腺瘤6例为单房囊性病灶,CT表现为单房囊低密度,边缘光整,无分叶,增强扫描壁强化不明显,不伴有壁结构包壳样钙化;8例为多房囊性病灶,CT表现肿瘤呈多房水样低密度,因内含粘液,其内密度不等。增强扫描后囊壁及分隔强化。结论MSCT检查可有效显示胰腺囊腺瘤的影像学特征,其CT表现具有一定的特征性,诊断准确率较高,表现典型者可作出明确诊断。

Objective To analyze the MSCT imaging manifestations of pancreatic cystadenoma and its differential diagnosis value.Methods The characteristics such as tumor size,shape,density and degree of enhancement were analyzed and the accuracy of MSCT examination in differential diagnosis of pancreatic cystadenomas was calculated.Results In 32 patients with pancreatic cystadenomas,the diagnostic accuracy of MSCT was 90.63%.Among 18 cases of pancreatic serous cystadenoma,the tumors were located in the pancreatic neck and neck of the pancreas in 7 patients,in the pancreas in 5 patients and in the tail of the pancreas in 6 patients.CT scan showed that the tumors in 7 cases were honeycomb with clear boundaries.CT of 3 cases showed fibrous central scars with characteristic calcification.After enhanced scanning,the honeycomb structure of the tumor became clearer.8 cases showed no scar in the center,and mural nodules can be seen in the capsule.CT showed unilocular cystic low-density shadows with lobulated edges and uniform internal density.There was no significant enhancement in the enhanced scan.Pancreatic mucinous cystadenoma was unilocular cystic lesion in 6 cases,and the CT manifestation showed unilocular cystic and low density,with smooth edges,no lobulation.Enhanced scans showed that enhancement wall was not significant,without claddinglike calcifications of wall structure.Eight cases showed multifocal cystic lesions,and CT showed that the tumors were plurilocular and was low density,which contained mucus,so it had different internal densities.After the scan,the capsule wall and the partition were strengthened.Conclusion MSCT examination can effectively display the imaging characteristics of pancreatic cystadenoma,and its CT manifestations have some characteristics,Its diagnostic accuracy is high,and those with typical manifestations can be diagnosed clearly.
