MRI加权联合动态增强对前列腺病变的诊断价值

Diagnostic Value of Diffusion-Weighted Imaging Combined with Dynamic Contrast-Enhanced MRI for Prostate Lesions

目的分析MRI加权(DWI)联合动态增强(DCE-MRI)对前列腺病变诊断价值。方法随机选取我院2016年1月至2019年1月收治的前列腺病变患者80例,分别接受DWI、DCE-MRI诊断。比较诊断结果评分、阳性值、诊断一致性及敏感度、特异度、准确度。结果DWI、DCE-MRI、DWI+DCE-MRI诊断结果评分差异比较有统计学意义(P<0.05)。评分由低到高依次是DWI、DCE-MRI、DWI+DCE-MRI。DWI、DCE-MRI、DWI+DCE-MRI阳性率差异比较有统计学意义(P<0.05)。阳性率由低到高依次是DWI、DCE-MRI、DWI+DCE-MRI。DWI、DCE-MRI、DWI+DCE-MRI的kappa值差异比较有统计学意义(P<0.05)。Kappa值由低到高依次是DWI、DCEMRI、DWI+DCE-MRI。DWI、DCE-MRI、DWI+DCEMRI敏感度、特异度、准确度差异比较有统计学意义(P<0.05)。敏感度由低到高依次是DCE-MRI、DWI、DWI+DCE-MRI;特异度、准确度由低到高依次是DWI、DCE-MRI、DWI+DCE-MRI。结论MRI加权联合动态增强对前列腺病变的敏感度、特异度、准确度较高,可提高诊断阳性率,有利于临床治疗。

Objective To investigate the diagnostic value of diffusion-weighted imaging(DWI)combined with dynamic contrast-enhanced MRI(DCE-MRI)for prostate lesions.Methods 80 patients with prostatic diseases admitted to our hospital from January 2016 to January 2019 were randomly selected and diagnosed by DWI and DCE-MRI respectively.The diagnostic scores,positive values,diagnostic consistency,sensitivity,specificity and accuracy of three examination methods were compared.Results The diagnostic score of DWI,DCE-MRI and DWI+DCE-MRI was significantly different(P<0.05),and was the highest in DWI+DCE-MRI,followed by DCE-MRI and DWI(P<0.05).The positive rate was the highest in DWI+DCE-MRI,followed by DCE-MRI and DWI(P<0.05).The kappa value of DWI,DCE-MRI and DWI+DCE-MRI was significantly different(P<0.05),and was the highest in DWI+DCE-MRI,followed by DCE-MRI and DWI(P<0.05).The sensitivity,specificity and accuracy of DWI,DCE-MRI and DWI+DCEMRI were significantly different(P<0.05).Sensitivity was the highest in DWI+DCE-MRI,followed by DWI and DCE-MRI(P<0.05).The specificity and accuracy were highest in DWI,highest by DCE-MRI and DWI+DCE-MRI(P<0.05).Conclusion The sensitivity,specificity and accuracy of DWI combined with DCEMRI for prostate lesions are relatively high,which can improve the positive rate of diagnosis.
