胺碘酮联合参松养心胶囊治疗慢性心力衰竭伴室性心律失常的临床研究

Clinical Study of Amiodarone Combined with Shensong Yangxin Capsules in Treatment of Chronic Heart Failure and Ventricular Arrhythmia

目的:探讨胺碘酮联合参松养心胶囊治疗慢性心力衰竭伴室性心律失常患者的临床效果。方法:2020年1月-2021年3月收治慢性心力衰竭患者120例,其中伴有室性心律失常患者60例(A组),无室性心律失常患者60例(B组)。将60例伴有室性心律失常患者分为两组,对照组给予常规治疗,观察组在常规治疗基础上给予胺碘酮联合参松养心胶囊治疗,比较两组治疗效果。结果:A组室性心律失常以短阵室速及室性早搏为主;A组心脏扩大、心肌功能严重受损发生率、脑钠肽浓度均高于B组;A组左心室射血分数低于B组。观察组治疗后左心室舒张末期内径低于对照组,左心室射血分数高于对照组;观察组治疗总有效率高于对照组,不良反应发生率低于对照组,差异有统计学意义(P<0.05)。结论:慢性心力衰竭患者室性心律失常发生率高,且其发生与心脏功能受损等情况有着密切关联,只有密切观察患者的临床症状和检查结果,并进行综合评价,及采取对症治疗,才能有效降低心律失常的发生率,提高治疗效果。

Objective:To investigate the clinical effect of amiodarone combined with Shensong Yangxin capsules on treating the patients with chronic heart failure and ventricular arrhythmia.Methods:From January 2020 to March 2021,120 patients with chronic heart failure were selected,including 60 patients with ventricular arrhythmia(group A)and 60 patients without ventricular arrhythmia(group B).Then 60 patients with ventricular arrhythmia were divided into the two groups.The control group was given conventional treatment.The observation group was given amiodarone combined with Shensong Yangxin capsules on the basis of conventional treatment.The treatment effects of the two groups were compared.Results:In the group A,ventricular arrhythmias were mainly short array ventricular tachycardia and ventricular premature beat.The incidences of cardiac enlargement,severe impairment of myocardial function and brain natriuretic peptide concentration in the group A were higher than those in the group B.The left ventricular ejection fraction in the group A was lower than that in the group B.The left ventricular end diastolic diameter in the observation group was lower than that in the control group,and the left ventricular ejection fraction was higher than that in the control group after treatment.The total effective rate of the observation group was higher than that in the control group,and the incidence of adverse reactions was lower than that in the control group,the differences were statistically significant(P<0.05).Conclusion:The incidence of ventricular arrhythmia in patients with chronic heart failure is high,and its occurrence is closely related to the impaired heart function.Only by closely observing the clinical symptoms and examination results of patients,making comprehensive evaluation and taking symptomatic treatment,the incidence of arrhythmias can be effectively reduced and the treatment effect can be improved.
