双歧杆菌三联活菌胶囊联合肠内营养支持在重症急性胰腺炎患者中的应用

Application of bifidobacterium triple viable capsule combined with enteral nutrition support in patients with severe acute pancreatitis

目的探讨双歧杆菌三联活菌胶囊联合肠内营养支持在重症急性胰腺炎(Severe acute pancreatitis, SAP)患者中的应用效果。方法选择2019年1月至2021年3月本院收治的SAP患者108例,按照治疗方案分为对照组和观察组各54例,对照组采用肠内营养支持,观察组采用双歧杆菌三联活菌胶囊联合肠内营养支持,比较两组疗效、腹痛、腹胀消失时间、排气功能恢复正常时间、治疗前后肠黏膜屏障功能[二胺氧化酶(DAO)、D-乳酸、内毒素(ET)]、炎症因子[肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)、C-反应蛋白(CRP)]及治疗期间并发症发生率。结果观察组总有效率高于对照组(P<0.05);观察组腹痛、腹胀消失时间、排气功能恢复正常时间短于对照组(P<0.05);治疗后观察组血清DAO、D-乳酸、ET水平低于对照组(P<0.05);治疗后观察组血清TNF-α、IL-6、CRP水平低于对照组(P<0.05);观察组并发症发生率低于对照组(P<0.05)。结论双歧杆菌三联活菌胶囊联合肠内营养支持治疗SAP患者效果显著,可有效改善肠黏膜屏障功能,缓解机体炎症,促使症状改善,降低并发症发生率。

Objective To explore the application effect of bifidobacterium triple viable capsule combined with enteral nutrition support in patients with severe acute pancreatitis(SAP). Methods A total of 108 patients with SAP in our hospital from January 2019 to March 2021 were collected, and divided into control group and observation group according to treatment regimens, with 54 cases in each group. The control group was treated with enteral nutrition support, and the observation group was treated with bifidobacterium triple viable capsule combined with enteral nutrition support. Curative effect, disappearance time of abdominal pain and abdominal distension, recovery time of exhaust function, intestinal mucosa barrier function[diamine oxidase(DAO), D-lactic acid and endotoxin(ET)], inflammation factors[tumor necrosis factor-α(TNF-α), interleukin 6(IL-6), C-reactive protein(CRP)] before and after treatment and incidence rate of complications during treatment were compared. Results The total effective rate of observation group was higher than that of control group(P<0.05). The disappearance time of abdominal pain and abdominal distension and recovery time of exhaust function in the observation group were shorter than those in the control group(P<0.05). After treatment, the serum levels of DAO, D-lactic acid and ET in observation group were lower than those in control group(P<0.05). After treatment, the serum levels of TNF-α, IL-6 and CRP in observation group were lower than those in control group(P<0.05). The incidence of complications in the observation group was lower than that in the control group(P<0.05). Conclusion The treatment of SAP patients with bifidobacterium triple viable capsule combined with enteral nutrition support has significant effect, which can effectively improve the intestinal mucosal barrier function, relieve the body inflammation, promote the improvement of symptoms and reduce the incidence of complications.
