早期限制性液体复苏对肝硬化上消化道出血的复苏成功率及预后影响

Influence on resuscitation successful rate and prognosis of early restriction fluid resuscitation for upper gastrointestinal bleeding in liver cirrhosis

目的观察早期限制性液体复苏对肝硬化上消化道出血复苏成功率及预后的影响。方法选择本院2018年2月至2019年3月收治的肝硬化所致上消化道出血患者90例,依据入院时间先后分为对照组及观察组各45例。两组均行无痛胃镜下止血,止血前对照组采用积极液体复苏,观察组实施早期限制性液体复苏,比较两组患者复苏成功率、完成止血时间、并发症发生情况。结果观察组患者的复苏成功率高于对照组(P<0.05);观察组患者输红细胞悬液量、止血时间、补液量及平均动脉压均低于对照组(P<0.05);观察组患者术后并发症发生率为低于对照组(P<0.05)。结论肝硬化上消化道出血患者应用早期限制性液体复苏,患者复苏成功率更高、止血效率更快,并发症发生率较低。

Objective To observe the influence on resuscitation successful rate and prognosis of early restriction fluid resuscitation for upper gastrointestinal bleeding in liver cirrhosis. Methods From February 2018 to March 2019, 90 patients with upper gastrointestinal bleeding caused by liver cirrhosis were selected. According to the time of admission, they were divided into control group and observation group, with 45 cases in each group. The two groups were treated with painless gastroscopy to stop bleeding. The control group was treated with aggressive fluid resuscitation before hemostasis. The observation group was treated with early restrictive fluid resuscitation. The success rate of resuscitation, the time to complete hemostasis and the occurrence of complications were compared between the two groups. Results The success rate of resuscitation of the observation group was higher than that of the control group(P<0.05). Erythrocyte suspension, hemostasis time, fluid replacement and mean arterial pressure in the observation group were lower than those in the control group(P<0.05). Incidence of postoperative complications of the observation group was lower than that in the control group(P<0.05). Conclusion Patients with upper gastrointestinal bleeding in liver cirrhosis were treated with early restrictive fluid resuscitation, the success rate of resuscitation was higher, the hemostatic efficiency was faster, and the incidence of complications was reduced.
