半导体激光应用于瘘道型根管再治疗的临床研究

摘要 目的探讨半导体激光在瘘道型根管再治疗中的临床效果。方法选择2021年1月至2021年8月本院收治的前牙需要根管再治疗的瘘道型根尖周炎患者124例,随机分为观察组和对照组各62例。观察组采用半导体激光处理后行常规热牙胶充填,对照组采用氢氧化钙糊剂进行消毒封药,1周后复诊,对于无急性炎症患者常规热牙胶充填,否则换药,直到无急性炎症后常规热牙胶充填,两周、1个月、3个月、6个月后复查,比较两组瘘道愈合时间、根尖阴影变化值及成功率。结果观察组治疗后两周、1个月患者瘘道愈合时间、根尖阴影变化值均短(少)于对照组(P<0.05);两组治疗后3个月及6个月治疗成功率差异无统计意义(P>0.05);观察组治疗后两周、1个月患者治疗成功率均高于对照组(P<0.05)。结论半导体激光用于瘘道型根管再治疗中能缩短操作及瘘道愈合时间,成功率较高,半导体激光根管消毒比氢氧化钙根管消毒更具有临床意义,值得推广应用。 Objective To investigate the clinical effect of semiconductor laser in the retreatment of fistula type root canal. Methods From January 2021 to August 2021, 124 patients with fistula periapical periodontitis who needed root canal retreatment were selected as subjects. The patients were randomly divided into observation group and control group, with 62 cases in each group. The observation group was filled with conventional hot gutta-percha after treatment with semiconductor laser, and the control group was filled with calcium hydroxide paste for disinfection and sealing. The follow-up visit was a week later. For patients with no acute inflammation, the conventional hot gutta-percha was filled, otherwise the dressing was changed until no acute inflammation, and then routine hot gutta-percha was filled. Re-examination was taken after two weeks, one month, three months, and six months later. The fistula healing time, apical shadow change value and success rate of the two groups were compared. Results Two weeks and one month after treatment, the healing time of the fistula and the change in apical shadow in the observation group were shorter(less) than those in the control group(P<0.05). The success rate at 3 and 6 months after treatment of two groups was not statistically significant(P>0.05). The success rates of the observation group at 2 weeks and 1 month after treatment were higher than those of the control group(P<0.05). Conclusion Semiconductor laser used in fistula type root canal retreatment can shorten the operation and fistula healing time, and the success rate is higher. Semiconductor laser root canal disinfection is more clinically significant than calcium hydroxide root canal disinfection, and it is worthy of popularization and application.
