微凸底盘造口袋联合宽胶带在肠造口回缩患儿中的应用效果

Application effect of micro-convex chassis ostomy bag combined with wide tape in children patients with enterostomy retraction

目的探讨微凸底盘造口袋联合宽胶带在肠造口回缩患儿中的应用效果。方法选取2017年1月—2020年12月收治的肠造口术后并发肠造口回缩的患儿40例,按照组间基线资料匹配的原则分成对照组与观察组,每组20例。观察组肠造口粪便收集采用50 mm微凸底盘两件式造口袋联合2.5 cm宽胶带封边增加稳固性,对照组采用一件式平面底盘儿童造口袋。跟踪评价1个月后两组患儿造口袋更换频次、肠造口周围皮肤DET评分[颜色改变(D)、浸渍/溃疡(E)、组织增生(T)]、肠造口周围皮肤刺激性皮炎发生率。结果两组患儿肠造口周围皮肤刺激性皮炎发生率比较差异无统计学意义(P>0.05);观察组造口袋更换频次低于对照组,组间比较差异有统计学意义(P<0.05);观察组肠造口周围皮肤DET评分低于对照组,组间比较差异有统计学意义(P<0.05)。结论肠造口粪便收集采用50 mm微凸两件式造口袋联合2.5 cm宽胶带封边虽不能明显降低肠造口回缩患儿肠造口周围皮肤刺激性皮炎的发生率,但可明显减少造口袋更换频次,降低肠造口周围皮肤损害程度。

Objective To application effect of micro-convex chassis ostomy bag combined with wide tape in children patients with enterostomy retraction.Methods To select 40 children patients who were hospitalized after colostomy and complicated with colostomy retraction from January 2017 to December 2020 as the study subjects.According to the matching principle of baseline data between groups,they were divided into a control group and an observation group,20 cases in each group.In the observation group,a 50mm slightly convex two-piece ostomy bag combined with a 2.5cm wide tape was used for feces collection to increase the stability,while the control group was used a one-piece children's ostomy bag.We followed up and evaluated the frequency of ostomy bag replacement,the DET score of the skin around the enterostomy[color change(D),maceration/ulcer(E),tissue hyperplasia(T)],and the incidence of irritant dermatitis in the two groups after 1 month.Results There was no significant difference in the incidence of irritant dermatitis around the enterostomy between the two groups(P>0.05).The frequency of ostomy bag replacement in the observation group was lower than that in the control group,and the diff erences between the groups were statistically signifi cant(P<0.05).The DET score of the skin around the intestinal stoma in the observation group was lower than that in the control group,and the diff erences between the groups were statistically signifi cant(P<0.05).Conclusion The application of 50mm slightly convex two-piece ostomy bag combined with 2.5cm wide tape for fecal collection cannot signifi cantly reduce the incidence of irritant dermatitis around enterostomy in children with colostomy retraction,but it can signifi cantly reduce the incidence of irritant dermatitis around the colostomy.The frequency of pocket replacement improved the degree of irritant dermatitis of the skin around the colostomy.
