摘要
目的探讨握拳旋腕运动预防淋巴瘤经外周静脉置入中心静脉导管(PICC)置管上肢深静脉血栓(DVT)的护理效果。方法选择医院血液内科2018年1—12月接收的PICC置管的淋巴瘤患者32例作为对照组,2019年1—12月接收的PICC置管淋巴瘤患者36例作为观察组。对照组实施常规护理,观察组在对照组基础上行握拳旋腕运动。比较两组患者上肢DVT发生率、其他导管相关并发症发生率;比较两组患者PICC置管前后的D-二聚体(D-D)水平。结果观察组置管28 d内的上肢DVT发生率低于对照组同期,差异有统计学意义(P<0.05)。观察组置管后28 d穿刺点出血、导管异位及机械性静脉炎发生率与对照组比较,差异无统计学意义(P>0.05)。置管14 d、置管28 d观察组的D-D水平低于对照组同期,差异有统计学意义(P<0.05)。结论握拳旋腕运动有助于维持淋巴瘤PICC置管后D-D水平稳定,降低PICC置管后上肢DVT发生率,且不增加其他导管相关并发症发生率。
Objective To explore the nursing effect of making a fist and rotating wrist exercise to prevent lymphoma peripherally inserted central venous catheters(PICC)catheterization deep venous thrombosis(DVT)in the upper limbs.Methods To select 32 lymphoma patients with PICC catheterization received in the Department of Hematology of the hospital from January to December 2018 as a control group,and 36 lymphoma patients with PICC catheterization received from January to December 2019 were selected as an observation group.The control group received routine nursing care,and the observation group performed making a fi st and rotating wrist exercises on the basis of the control group.The incidence of DVT in the upper limbs and the incidence of other catheter-related complications were compared between the two groups;the D-dimer(D-D)levels before and after PICC catheterization were compared between the two groups.Results The incidence of upper limb DVT within 28 days of catheterization in the observation group was lower than that in the control group during the same period,and the diff erence was statistically signifi cant(P<0.05).There was no statistically signifi cant diff erence in the incidence of puncture point bleeding,catheter ectopic position and mechanical phlebitis in the observation group 28 days after catheter placement and the control group(P>0.05).The D-D level of the observation group of 14d and 28d catheterization was lower than that of the control group during the same period,and the diff erence was statistically signifi cant(P<0.05).Conclusion Making a fi st and rotating wrist exercise can help maintain the stability of D-D level after PICC catheterization in lymphoma,reduce the incidence of DVT in the upper limbs after PICC catheterization,and does not increase the incidence of other catheter-related complications.
