细胞免疫疗法联合腹腔镜胃癌根治术对胃癌患者免疫功能及生活质量的影响

Effects of cellular immunotherapy combined with laparoscopic radical gastrectomy on immune function and quality of life in patients with gastric cancer

目的探讨细胞免疫疗法联合腹腔镜胃癌根治术对胃癌患者免疫功能及生活质量的影响。方法选取2018年6月至2020年6月收治的50例胃癌患者作为研究对象,采用随机数字表法将其分为对照组(25例,腹腔镜胃癌根治术+常规化疗)和观察组(25例,腹腔镜胃癌根治术+常规化疗+细胞免疫疗法)。比较两组的临床疗效、肿瘤干细胞标志物水平、免疫功能指标、不良反应发生情况及生活质量。结果观察组的治疗总有效率为88.0%,明显高于对照组的60.0%,差异具有统计学意义(P<0.05)。治疗前,两组的CD133、CD44、CD168水平比较,差异无统计学意义(P>0.05);治疗后,观察组的CD133、CD44、CD168水平明显低于对照组,差异具有统计学意义(P<0.05)。治疗前,两组的CD4^(+)、CD8^(+)、CD3^(+)、NK比较,差异无统计学意义(P>0.05);治疗后,观察组的CD4^(+)、CD3^(+)、NK明显高于对照组,CD8^(+)明显低于对照组,差异具有统计学意义(P<0.05)。观察组的不良反应总发生率为12.0%,明显低于对照组的36.0%,差异具有统计学意义(P<0.05)。观察组的情感功能、角色功能、躯体功能、认知功能、社会功能、总体健康状况评分高于对照组,差异具有统计学意义(P<0.05)。结论细胞免疫疗法联合腹腔镜胃癌根治术治疗胃癌的效果显著,可增强机体的免疫功能,进而提高患者的生活质量,值得临床推广和应用。

Objective To explore the effects of cellular immunotherapy combined with laparoscopic radical gastrectomy on immune function and quality of life in patients with gastric cancer.Methods A total of 50 patients with gastric cancer treated from June 2018 to June 2020 were selected as the research objects.The patients were divided into control group(25 cases,laparoscopic radical gastrectomy+conventional chemotherapy)and observation group(25 cases,laparoscopic radical gastrectomy+conventional chemotherapy+cellular immunotherapy)by random number table method.The clinical efficacy,cancer stem cell markerslevels,immune function indexes,occurrence of adverse reactions and quality of life were compared between the two groups.Results The total effective rate of treatment in the observation group was 88.0%,which was significantly higher than 60.0%in the control group,and the difference was statistically significant(P<0.05).Before treatment,there were no significant differences in the levels of CD133,CD44 and CD168 between the two groups(P>0.05);after treatment,the levels of CD133,CD44 and CD168 in the observation group were significantly lower than those in the control group,and the differences were statistically significant(P<0.05).Before treatment,there were no significant differences in CD4^(+),CD8^(+),CD3^(+)and NK between the two groups(P>0.05);after treatment,CD4^(+),CD3^(+)and NK in the observation group were significantly higher than those in the control group,and CD8^(+)was significantly lower than those in the control group,the differences were statistically significant(P<0.05).The total incidence of adverse reactions in the observation group was 12.0%,which was significantly lower than 36.0%in the control group,and the difference was statistically significant(P<0.05).The scores of emotional function,role function,physical function,cognitive function,social function and overall health status in the observation group were higher than those in the control group,and the difference was statistically significant(P<0.05).Conclusion Cellular immunotherapy combined with laparoscopic radical gastrectomy in the treatment of gastric cancer has significant effect,which can enhance the immune function of the body and improve the quality of life of patients.It is worthy of clinical promotion and application.
