卡前列甲酯栓联合缩宫素防治产后出血的临床报告

Clinical report of carboprost methyl suppository combined with oxytocin in the prevention and treatment of postpartum hemorrhage

目的探讨在防治产后出血中,联合应用卡前列甲酯栓和缩宫素的临床效果分析。方法选取2020年4月1日-2021年4月30日本院收治的80例孕妇作为研究对象,按照随机数表法分为对照组和观察组两组,每组各40例。对照组用宫缩素防治,观察组联合应用卡前列甲酯栓和缩宫素防治。结果观察组总有效率为97.50%(39/40),明显比对照组的80.00%(32/40)(P=0.0133)。观察组患者2小时出血量(126.63±36.66)ml及24小时出血量(173.57±46.25)ml,均比对照组2小时出血量(236.15±51.37)ml及24小时出血量(278.71±52.90)ml显著降低(均P<0.05);此外,观察组第三产程时间(8.15±3.30)h相较于对照组第三产程时间(14.12±3.24)h显著缩短(P=0.0032)。观察组患者总不良事件发生率7.50%(3/40),显著低于照组的27.50%(11/40)(P=0.0367)。用药前与用药后30 min,两组血压水平差异均无统计学意义(P>0.05)。与对照组相较生活品质评分,观察组更为良好(P<0.05)。结论在防治产后出血中,联合应用卡前列甲酯栓和缩宫素,可使产后出血量有所降低,第三产程时间缩短,不良事件发生情况有所减少,同时能改善患者的血压水平,提升患者的生存质量,推广和运用价值显著。

Objective To investigate the clinical effects of carboprost methyl suppository combined with oxytocin in the prevention and treatment of postpartum hemorrhage.Methods A total of 80 pregnant women those were admitted to our hospital from April 1,2020 to April 30,2021 were selected as the research objects.According to the random number table method,the patients were divided into two groups of control group and and observation group,with 40 patients in each group.The control group was treated with oxytocin,and the observation group was treated with carboprost methyl suppository combined with oxytocin.Results The total effective rate of the observation group was 97.50%(39/40),it was significantly higher than 80.00%(32/40)in the control group(P=0.0133).The 2-hour blood loss(126.63±36.66)ml and 24-hour blood loss(173.57±46.25)mL of the observation group were significantly lower than the 2-hour blood loss(236.15±51.37)ml and 24-hour blood loss(278.71±52.90)ml of the control group(both P<0.05).In addition,the time of the third stage of labor in the observation group(8.15±3.30)was significantly shorter than(14.12±3.24)of the control group(P=0.0032).The incidence of total adverse events in the observation group(3/40,7.50%)was significantly lower than that(11/40,27.50%)in the control group(P=0.0367).There was no significant difference in blood pressure levels between the two groups before medication and 30 minutes after the medication(P>0.05).Compared with the control group,the observation group had better quality of life scores(P<0.05).Conclusions The combination of carboprost methyl suppository and oxytocin in the prevention and treatment of postpartum hemorrhage could reduce the amount of postpartum hemorrhage,shorten the time of the third stage of labor,and reduce the occurrence of adverse events.At the same time,it could improve the patient's blood pressure level and improve the patient's quality of life.It is of value for promotion and application significantly.
