基于大数据平台探讨“益肾清利活血方”治疗慢性肾脏病1-2期蛋白尿的临床疗效分析

Clinical Effect Analysis of Yishen Qingli Huoxue Recipe on Proteinuria of Chronic Kidney Disease 1-2 Stages Based on Big Data Platform

目的利用"中国肾脏病大数据应用创新联盟大数据平台"(以下简称"中国肾脏病大数据平台")提取真实世界数据,观察"益肾清利活血方"对慢性肾脏病1-2期蛋白尿的治疗作用。方法收集2015年1月1日至2020年6月1日于"中国肾脏病大数据应用平台"随访超过12个月的慢性肾脏病1-2期非肾病范围内蛋白尿患者共125例,比较治疗6个月、治疗12个月的24 h尿蛋白定量及疗效分析。结果与治疗前比较,治疗6个月后总有效率为65.6%,经治12个月后总有效率为75.2%;治疗6个月后的24 h尿蛋白定量水平(0.96 g)较基线水平(1.22 g)下降(P<0.05),治疗12个月后的24 h尿蛋白定量(0.77 g)显著下降(P<0.01),控制蛋白尿疗效随着疗程延长而增加。此外,我们观察到其中32例患者有治疗前后肾功能对比,其中尿素氮、血肌酐较治疗前均有所下降,估算的肾小球滤过率有所上升,但均无统计学差异,即患者肾功能保持相对稳定。结论"益肾清利活血方"可以降低慢性肾脏病1-2期患者尿蛋白水平、维持肾功能稳定,值得临床推广。

Objective Based on the real-world study,to extract data from“China Kidney disease Big data Application platform”,to observe the therapeutic effect of Professor Sun Wei’s“Yishen Qingli Huoxue Recipe”on proteinuria of chronic kidney disease 1-2 stages.Methods A total of 125 patients with proteinuria of CKD1-2 stages were admitted to the Department of Nephrology,Jiangsu Province Hospital of Traditional Chinese Medicine from Jan 1,2015 to Jun 1,2020,whom were observed over 12 months.After treatments,the 24 h UTP and BUN,Scr were measured and analyzed.Results The total effective rate was 65.6%after 6 months treatments,as well as was 75.2%after 12 months treatments.Compared with the baseline,the 24 h UTP was declined from 1.22 g to 0.96 g after 6 months′treatment(P<0.05),and decline further to 0.77 g after 12 months′treatments(P<0.01).Otherwise,we observed renal function of 32 patients with integrated records.The BUN,Scr were decreased while e GFR was increased after treatments without statistical significance.Conclusion“Yishen Qingli Huoxue Recipe”could reduce the 24 h UTP,and stabilize renal function in patients with CKD 1-2 stages.It is worth promoting applications in clinic.
