基于“益肾清利活血法”治疗的慢性肾脏病3期病人的转归分析

Outcome Analysis of Patients with Stage 3 Chronic Kidney Disease Treated with"Kidney-Tonifying Blood-Activating Method"

目的研究经过"益肾清利活血法"治疗的慢性肾脏病3期(Chronic Kidney Disease 3,CKD3)期患者分期转归情况,分析其与患者临床特点的相关性。方法纳入符合要求的CKD分期逆转组和未逆转组患者共69例,对两组患者一般情况、临床特点及随诊情况等比较分析。结果(1)两组患者一般情况无统计学差异,但逆转组患者就诊时肾损伤指标较低、蛋白尿更少、贫血及肾萎缩程度较轻,差异有统计学差异。(2)中医证候方面,两组患者就诊时本虚证无明显差异,邪实证中逆转组患者以湿浊证为主,未逆转组以湿热证为主。(3)两组患者随诊情况比较,未逆转组加入慢病管理患者比例更高,逆转早的患者逆转后维持时间更长,且具有统计学意义。结论孙伟教授"益肾清利活血法"可以有效延缓和逆转CKD3期患者肾脏病分期进展,但因患者临床特点及随诊情况不同而有差异。

Objective To study the stage outcome of chronic kidney disease stage 3(CKD3)patients treated by"Tonifying kidney,clearing blood and promoting blood circulation method",and to analyze its correlation with the clinical characteristics of patients.Methods A total of 69 patients who met the requirements of CKD stage reversal group and non-reversal group were included.The general condition,clinical characteristics and follow-up of the two groups were compared and analyzed.Results(1)There was no statistical difference in general conditions between the two groups,but patients in the reversal group had lower renal injury indexes,less proteinuria,less anemia and less renal atrophy,and the differences were statistically significant.(2)In terms of TCM syndromes,there was no significant difference between the two groups of patients in the treatment of this deficiency syndrome.In the evil demonstration,patients in the reverse group were mainly characterized by dampness and turbidity,while those in the non-reverse group were mainly characterized by dampness and heat.(3)Comparison of patients′follow-up between the two groups showed that the proportion of patients with chronic disease management in the non-reversal group was higher,and patients with early reversal lasted longer after reversal,which was statistically significant.Conclusion Professor Sun Wei′s method of"Kidney-Tonifying Blood-Activating Method"can effectively delay and reverse the stage progression of kidney disease in patients with CKD3,but there are differences due to different clinical characteristics and follow-up of patients.
