基于数据挖掘的血液病从虚劳论治的证治规律研究

Study on Syndrome and Treatment Law of Treatment of Hematopathy Induced Deficiency Based on Data Mining

目的探讨中医药从虚劳论治血液病的证治规律,为临床治疗的运用与相关药物研发提供必要的参考依据。方法通过检索CNKI、CBM、维普、万方等数据库,收集整理相关文献,建立中医治疗虚劳性血液病证治信息数据库。运用Excel 2013软件进行描述性分析,以R语言软件和Python软件开展关联规则和聚类分析。结果血液病的主要症状为乏力、头晕、心悸、气短、腰膝酸软等等,舌象主要表现为舌淡、舌红、苔薄、苔白等等,脉象主要表现为脉细、脉数、脉沉、脉弱等等。从虚劳论治血液病最常使用的药物为补气药类、补血药类、滋阴药类和活血化瘀药类。用药以温平为主,药味以甘味(49.99%)、苦味(21.24%)和辛味(12.68%)最为常用。常用的补气药类有黄芪、白术、党参和茯苓等,常用的补血药类有当归、熟地黄、鸡血藤和白芍等,常用的滋阴药类有熟地黄、女贞子、阿胶和墨旱莲等,常用的活血化瘀药类有生地黄、丹参、牡丹皮和赤芍等。关联支持度最高为黄芪和人参,其次为女贞子和墨旱莲,符合气虚和阴虚的病机。聚为4类,以补虚药和活血药两类为主。结论血液病从虚劳论治,症状舌脉符合气虚和阴虚病机,治疗以补气滋阴药物为主。

Objective To investigate the regularity of syndrome and treatment in treating deficiency induced by hematopathy(DIH),so as to provide the necessary reference for the application of clinical treatment and the research and development of related medicines.Methods The databases including Chinese National Knowledge Infrastructure(CNKI),Chinese Biomedical Literature Database(CBM),VIP and Wanfang were searched to collect the related literatures,and a database of syndromes and treatments of TCM treatment of DIH was established.Excel 2013 software was used for descriptive analysis,and R language software and Python software were used for association rules and cluster analysis.Results The main symptoms of DIH were weakness,dizziness,palpitation,shortness of breath,weakness of waist and knee,etc.The main manifestations of tongue image were light tongue,red tongue,thin fur,white fur and so on.The main manifestations of pulse image were fine pulse,rapid pulse,deep pulse,weak pulse and so on.The main medicines of treating DIH were qi-invigorating medicine,blood-replenishing medicine,yin-nourishing medicine and blood-activating and stasis-removing medicine.The most commonly used medicines were warm and flat.The flavors of commonly used medicines were sweet(49.99%),bitter(21.24%)and pungent(12.68%).The main medicine of qi-invigorating medicine were Astragali Radix,Atractylodis Macrocephalae Rhizoma,Codonopsis Pilosula,Poria and so on.Blood-replenishing medicine included Angelicae Sinensis Radix,Rehmanniae Radix Praepapata,Spatholobi Caulis,Paeoniae Radix Alba,etc.;Yin-nourishing medicine included Rehmanniae Radix Praepapata,Ligustri Lucidi Fructus,Donkey Hide Gelatin,Eclipta Sinensis,etc.;Blood-activating and stasis-removing medicine included Rehmanniae Radix,Salvia Mil,Peony Bark,Radix Paeoniae Rubra,etc.The highest support of association rule was Astragali Radix and Ginseng Radix Et Rhizoma.The second was Ligustri Lucidi Fructus and Eclipta Sinensis,which met the pathogenesis of qi deficiency and yin deficiency.It was divided into four categories,mainly including medicine of tonifying deficiency and activating blood circulation.Conclusion The symptoms of DIH of tongue and pulse accord with the pathogenesis of deficiency of both qi and yin,and the treatment is mainly based on the medicine of tonifying qi and nourishing yin.
