醋甘遂联合大承气汤治疗重症急性胰腺炎的疗效观察

Observation on the curative effect of vinegar Gansui combined with Dachengqi Decoction on severe acute pancreatitis

目的:观察用醋甘遂联合大承气汤对重症急性胰腺炎(SAP)患者进行治疗的临床效果。方法:择取2018年4月至2020年3月常熟市中医院脾胃肝胆科接诊的SAP患者43例,将其随机分为对照组和观察组。对两组患者均进行常规治疗。在此基础上,为对照组患者采用大承气汤进行治疗,为观察组患者采用醋甘遂联合大承气汤进行治疗。然后比较两组患者的治疗效果。结果:观察组患者治疗的总有效率(90.91%)高于对照组患者治疗的总有效率(61.90%),P<0.05。治疗后,观察组患者腹胀缓解的时间、腹痛缓解的时间、恢复排便的时间、住院的时间均短于对照组患者,P<0.05。治疗3 d后和治疗5 d后,两组患者的急性生理学与慢性健康状况(APACHE)Ⅱ评分均显著低于治疗前,且观察组患者的APACHEⅡ评分均低于对照组患者,P<0.05。结论:用醋甘遂联合大承气汤治疗SAP可取得较为理想的疗效,能够快速地缓解患者的临床症状,促进其康复。

Objective:To observe the clinical effect of treating severe acute pancreatitis(SAP)with Vinegar Gansui combined with Dachengqi Decoction.Methods:A total of 43 SAP patients admitted to the department of Spleen,stomach,hepatobiliary and hepatobiliary diseases of Changshu Hospital of Traditional Chinese Medicine from April 2018 to March 2020 were selected and randomly divided into control group and observation group.Routine treatment was performed in both groups.On this basis,patients in the control group were treated with Dachengqi Decoction,patients in the observation group were treated with Vinegar gansui combined with Dachengqi Decoction.Then the treatment effects of the two groups were compared.Results:The total effective rate of observation group(90.91%)was higher than that of control group(61.90%),P<0.05.After treatment,the time of abdominal distention relief,abdominal pain relief,recovery of defecation time and hospitalization time in observation group were shorter than those in control group,P<0.05.After 3 d of treatment and 5 d of treatment,the scores of acute physiology and chronic health status(APACHE)ⅱin 2 groups were significantly lower than before treatment,and the APACHEⅱscores in observation group were lower than control group,P<0.05.Conclusion:The treatment of SAP with Vinegar gansu combined with Dachengqi decoction can achieve ideal curative effect,can quickly relieve the clinical symptoms of patients,and promote their recovery.
