医护一体化加速康复外科理念对超低位直肠癌患者术后康复及视觉模拟评分法评分的影响分析

The Influence of Enhanced Recovery after Surgery Concept of Integrated Medical Care on Postoperative Rehabilitation and VAS Score of Patients with Ultra-low Rectal Cancer

目的:探讨医护一体化加速康复外科(ERAS)理念对超低位直肠癌患者术后康复及视觉模拟评分法(VAS)评分的影响。方法:选取漯河市郾城区人民医院2018年5月―2020年8月收治的86例超低位直肠癌术后患者作为研究对象,按照不同入院顺序分为对照组和联合组,每组各43例。对照组采用常规护理,联合组予以医护一体化ERAS理念护理。比较两组患者术后康复情况、肛门功能恢复情况及干预前后VAS评分。结果:联合组患者首次排气时间、下床活动时间、进食时间、住院时间短于对照组,差异有统计学意义(t=5.382、3.676、3.075、12.494,P<0.05);联合组患者肛门功能恢复优良率为93.02%(40/43),高于对照组的72.09%(31/43),差异有统计学意义(χ^(2)=6.541,P<0.05);干预后联合组患者疼痛程度低于对照组,差异有统计学意义(μ=3.245,P<0.05);联合组患者护理满意度为95.35%,高于对照组的74.42%,差异有统计学意义(χ^(2)=7.340,P<0.05)。结论:基于ERAS理念的医护一体化护理能加速超低位直肠癌患者术后康复,改善其肛门功能恢复情况,降低患者疼痛程度,提高护理满意度。

Objective:To explore the effect of the concept of enhanced recovery after surgery(ERAS)of integrated medical care on the postoperative rehabilitation and visual analogue scale(VAS)scores of patients with ultra-low rectal cancer.Methods:A total of 86 postoperative patients with ultra-low rectal cancer in the hospital from May 2018 to August 2020 were selected as the research objects and divided into control group and combination group according to different admission orders,with 43 cases in each group.The control group was given routine nursing care,and the combination group was given the ERAS concept of integrated medical care.The postoperative rehabilitation,anal function recovery,and VAS scores before and after intervention were compared between the two groups.Results:The first exhaust time,the time to get out of bed,the time to eat,and the length of hospital stay in the combination group were shorter than those in the control group,with statistically significant difference(t=5.382,3.676,3.075,12.494,P<0.05).The excellent rate of recovery of anal function in the combination group was 93.02%(40/43),higher than72.09%(31/43)of the control group,with statistically significant difference(χ^(2)=6.541,P<0.05).After the intervention,the pain degree of the combination group was lower than that of the control group,with statistically significant difference(μ=3.245,P<0.05).The nursing satisfaction degree of the combined group was 95.35%,higher than 74.42%of the control group,with statistically significant difference(χ^(2)=7.340,P<0.05).Conclusion:The integrated medical and nursing care based on the concept of ERAS can accelerate the postoperative recovery of patients with ultra-low rectal cancer,improve the recovery of their anal function,reduce the pain of patients,and improve nursing satisfaction.
