层级整体护理模式在肾结石取石术后造瘘管患者中的应用效果观察

Effective Application of Hierarchical Holistic Nursing Model in Patients with Fistula after Nephriolithotomy

目的:分析在肾结石取石术后造瘘管患者的临床护理中,应用层级整体护理模式的护理效果。方法:将漯河市中心医院2019年6月―2020年6月收治的100例肾结石取石术后造瘘管患者作为研究对象,通过双盲选法将其分为对照组和观察组,每组各50例。对照组开展常规护理,观察组以层级整体护理模式展开护理,比较两组患者护理后的拔管时间、尿液转清时间、住院时间、并发症总发生率以及不良事件总发生率,同时观察两组患者护理后的临床护理质量以及护理满意度。结果:相比于对照组,观察组的拔管时间明显更短、尿液转清时间明显更短、住院时间明显更短、并发症总发生率明显更低和不良事件总发生率明显更低,差异有统计学意义(t=4.199、14.697、4.046,P<0.05;χ^(2)=4.332、4.762,P<0.05)。相比于对照组,观察组在基础护理、安全管理及护理文书书写方面的评分均明显更高,护理质量明显更高,同时观察组护理满意度评分明显更高,差异有统计学意义(t=6.761、20.133、8.815、13.394,P<0.05)。结论:在肾结石取石术后造瘘管患者的临床护理中,层级整体护理模式具有良好的护理应用价值,可以有效减少各种不良事件以及并发症的发生,有利于进一步加快患者术后康复,提高临床护理质量及护理满意度。

Objective:To analyze the nursing effect of applying hierarchical holistic nursing mode in the clinical nursing of patients with fistula after nephriolithotomy.Methods:A total of 100 patients with fistula after renal stone removal in the hospital from June 2019 to June 2020 were selected as the research objects,and they were divided into control group and observation group by double-blind selection method,with 50 cases in each group.The control group received routine nursing,while the observation group received hierarchical holistic nursing mode.The differences in extubation time,urine clearing time,length of hospital stay,total incidence of complications and total incidence of adverse events were compared between the two groups after nursing,and the clinical nursing quality and nursing satisfaction of the two groups after nursing were observed.Results:Compared with the control group,the observation group had significantly shorter extubation time,shorter urine cleaning time,shorter hospital stay,significantly lower overall incidence of complications and adverse events,with statistically significant difference(t=4.199,14.697,0.046,P<0.05,χ^(2)=4.332,4.762,P<0.05).Compared with the control group,the scores of basic nursing,safety management and nursing document writing in the observation group were significantly higher,and the nursing quality was significantly higher.Meanwhile,the score of nursing satisfaction in the observation group was significantly higher,with statistically significant difference(t=6.761,20.133,8.815,13.394,P<0.05).Conclusion:For patients with ostomy cannula after renal stone removal,in their clinical nursing,the hierarchical holistic nursing model has good nursing application value,which can effectively reduce the occurrence of various adverse events and complications,and is conducive to further speeding up the postoperative recovery of patients and improving clinical nursing quality and nursing satisfaction.
