急诊重症监护病房心肺复苏效果的相关因素分析

Related Factors Analysis of Cardiopulmonary Resuscitation in Emergency Intensive Care Unit

目的 研究在急诊重症监护病房(EICU)接受心肺复苏(CPR)患者中,复苏成功的相关因素分析。方法 选取018年1月至2019年6月滨州医学院附属医院EICU住院过程中出现心跳呼吸骤停并进行CPR的患者,共117例作为本次研究对象。以CPR后出现自主循环恢复(ROSC)并存活出院为心肺复苏成功标准,分析影响EICU内心肺复苏效果的相关因素。结果 纳入统计的117例患者中,33例最终复苏成功,占28.21%。经单因素分析,心肺复苏持续时间(15.152±10.456)min、肾上腺素用量(2.303±1.132)mg;在心跳呼吸骤停前是否已气管插管及心跳呼吸骤停前是否应用血管活性药物在影响心肺复苏成功率方面均存在不同的差异;而心功能衰竭、呼吸衰竭、肾功能不全各方面两组之间的影响EICU心肺复苏效果的单因素数据,差异有统计学意义(P<0.05)。将单因素有意义的变量拟合多因素方程,P=0.15为剔除标准,采用后退法筛选变量,得出CPR持续时间和肾上腺素用量是影响EICU心肺复苏效果的因素。结论 在急诊重症监护病房(EICU)接受心肺复苏(CPR)患者中,患者的心肺复苏持续时间、肾上腺素用量、心跳呼吸骤停前是否已气管插管及是否应用血管活性药物影响心肺复苏成功率。

Objective To study related factors of successful resuscitation of patients with cardiopulmonary resuscitation (CPR) in emergency intensive care unit (EICU).Methods Initial time of the study was January 2018 and deadline was June 2019.The paper chose 117 cases cardiac and respiratory arrest and CPR patients during hospitalization in EICU of our hospital as study objects,analyzed related factors affecting effect of cardiopulmonary resuscitation in EICU with success cardiopulmonary resuscitation standard of spontaneous circulation (ROSC) recovery after CPR,survival and discharge.Results Among 117 cases included in the statistics,33 cases were successfully resuscitated,accounting for 28.21%.By univariate analysis,duration of cardiopulmonary resuscitation was (15.152±10.456) min and adrenaline dosage was (2.303±1.132) mg;whether endotracheal intubation had been carried on before cardiac and respiratory arrest and whether vasoactive drugs been applied before cardiac and respiratory arrest showed different effect on success rate of cardiopulmonary resuscitation;single factor data of heart failure,respiratory failure and renal insufficiency affecting effect of cardiopulmonary resuscitation in EICU was significantly different between two groups (P<0.05).Univariate meaningful variables were fitted to multivariate equation with elimination standard of P=0.15.The paper screened variables with regression method,and concluded CPR duration and adrenaline dosage were factors affecting effect of EICU cardiopulmonary resuscitation.Conclusion Duration of cardiopulmonary resuscitation,adrenaline dosage,whether endotracheal intubation been carried on before cardiac and respiratory arrest and whether vasoactive drugs applied may affect success rate of cardiopulmonary resuscitation.of patients with cardiopulmonary resuscitation (CPR) in emergency intensive care unit (EICU).
