摘要
目的 分析探究超声心动图鉴别诊断扩张型心肌病(DCM)与心室肌致密化不全(NVM)的临床价值。方法 选择50例患者展开回顾性分析,划分为DCM组和NVM组,以上50例患者均接受超声心动图进行相关检查,并针对两组患者的影像学资料进行分析比较。结果 ①两组超声心动图表现:两组患者LVEF水平无显著差异,不具备统计学意义(P>0.05);DCM组的心内膜最大厚度、左室扩大、致密化心外膜层厚度比值以及室壁厚度显著低于NVM组,差异具有统计学意义(P<0.05);②超声心动图鉴别诊断DCM的ROC曲线检验结果:将DCM作为本次研究的状态变量,采用ROC曲线进行分析显示AUC值均>0.7,结果提示超声心动图鉴别诊断DCM具有极高的诊断准确率。结论 不论是对于扩张型心肌病还是心室肌致密化不全的临床诊断均可选择超声心动图进行检查,其具有诊断准确率高、能够有效鉴别疾病类型的特点,可为临床医师诊疗疾病、预估预后提供重要的参考依据,值得推广应用。
Objective To analyze the clinical value of echocardiography in the differential diagnosis of dilated cardiomyopathy (DCM) and ventricular noncompaction (NVM).Methods Fifty patients were selected for retrospective analysis and divided into DCM group and NVM group.The above 50 patients were examined by echocardiography,and the imaging data of the two groups were analyzed and compared.Results ① Echocardiographic performance of the two groups:There was no significant difference in LVEF levels between the two groups,which was not statistically significant (P>0.05).The maximum endocardial thickness,left ventricular enlargement,densified epicardial layer thickness ratio,and ventricular wall thickness in the DCM group were significantly lower than those in the NVM group,and the difference was statistically significant (P<0.05);② ROC curve test results for differential diagnosis of DCM by echocardiography:Taking DCM as the state variable in this study,ROC curve analysis showed that the AUC values were all>0.7,and the results suggest that echocardiography has extremely high diagnostic accuracy in the differential diagnosis of DCM.Conclusion Whether for the clinical diagnosis of dilated cardiomyopathy or ventricular noncompaction,echocardiography can be selected for examination.It has the characteristics of high diagnostic accuracy and effective differentiation of disease types.It can provide important reference for clinicians to diagnose and treat diseases and predict prognosis,and is worthy of popularization and application.
