鼻内镜鼻窦手术联合低温等离子下鼻甲射频消融术治疗慢性鼻-鼻窦炎患者的效果分析 被引量:1

摘要 目的 分析鼻内镜鼻窦手术联合低温等离子下鼻甲射频消融术治疗慢性鼻-鼻窦炎(CRS)患者的效果。方法 将江门市中心医院江海分院2019年1月-2020年4月收治的CRS患者48例以随机数字表法分为两组,对照组24例,行鼻内镜鼻窦手术,观察组24例,在对照组基础上联合低温等离子下鼻甲射频消融术,观察两组患者手术一般情况、肺通气功能、鼻黏液纤毛清除功能。结果 观察组手术时间、住院时间短于对照组,术中出血量少于对照组(P<0.05),术后观察组第1秒用力呼气容积(FEV1)、最大呼气流量(PEF)、用力肺活量(FVC)、第1秒用力呼气容积占用力肺活量比值(FEV1/FVC)均高于对照组(P<0.05),观察组黏液纤毛清除率(MTR)高于对照组,糖精清除时间(SCT)、黏液纤毛清除速率(MCT)低于对照组(P<0.05)。结论 鼻内镜鼻窦手术联合低温等离子下鼻甲射频消融术治疗CRS,可以缩短手术时间和住院时间,减少术中出血量,改善鼻腔黏液纤毛清除率,提高肺通气功能。 Objective To analyze the effect of endoscopic sinus surgery combined with low-temperature plasma radiofrequency ablation of inferior turbinate in the treatment of patients with chronic rhinosinusitis (CRS).Methods A total of 48 patients with CRS admitted to Jianghai Branch of Jiangmen Central Hospital (Jianghai District People’s Hospital) from January 2019 to April 2020 were divided into two groups by random number table method.24 patients in the control group received endoscopic sinus surgery,and 24 patients in the observation group received radiofrequency ablation of inferior turbinate combined with low-temperature plasma on the basis of the control group.The general situation of operation,pulmonary ventilation function and nasal mucociliary removal function were observed in 2 groups.Results The operation time and hospitalization time of the observation group were shorter than those of the control group,and the amount of intraoperative blood loss was less than that of the control group (P<0.05).The forced expiratory volume in the first second (FEV1),maximum expiratory flow (PEF),forced expiratory volume (FVC) and the ratio of forced expiratory volume (FEV1 /FVC) in the first second in the observation group were higher than those in the control group (P<0.05),and the mucociliary clearance rate (MTR) in the observation group was higher than that in the control group.Saccharin clearance time (SCT) and mucociliary clearance rate (MCT) were lower than those in the control group (P<0.05).Conclusion Endoscopic sinus surgery combined with low-temperature plasma radiofrequency ablation of inferior turbinate in the treatment of CRS can shorten the duration of surgery and hospital stay,reduce intraoperative blood loss,improve nasal mucociliary clearance rate,and improve pulmonary ventilation function.
