经尿道膀胱肿瘤电切术与传统开放手术治疗膀胱癌的疗效比较

Comparison on the efficacy of transurethral resection of bladder tumor and traditional open surgery in the treatment of bladder cancer

目的分析经尿道膀胱肿瘤电切术与传统开放手术治疗膀胱癌的疗效。方法60例膀胱癌患者,采用随机数字表法分为对照组和研究组,各30例。对照组接受传统开放手术治疗,研究组接受经尿道膀胱肿瘤电切术治疗。对比两组手术、住院指标及术后视觉模拟评分法(VAS)评分、术后并发症发生情况、术后复发情况。结果研究组术中出血量为(52.29±10.53)ml,少于对照组的(94.39±28.67)ml,差异具有统计学意义(P<0.05);研究组手术时间为(45.31±7.92)min,拔管时间为(2.14±1.03)d,住院时间为(7.03±1.17)d,均短于对照组的(77.71±17.51)min、(4.57±1.65)d、(13.64±3.55)d,差异具有统计学意义(P<0.05)。研究组术后VAS评分(2.67±0.98)分低于对照组的(4.36±1.15)分,差异具有统计学意义(t=6.126,P=0.000<0.05)。两组术后并发症发生率对比差异无统计学意义(χ^(2)=1.964,P=0.161>0.05)。两组术后复发率对比差异无统计学意义(χ^(2)=0.351,P=0.554>0.05)。结论经尿道膀胱肿瘤电切术治疗膀胱癌能够达到与开放性手术相同的治疗效果,且具有微创优势,患者术后疼痛轻、出院早、恢复快。

Objective To analyze the efficacy of transurethral resection of bladder tumor and traditional open surgery in the treatment of bladder cancer.Methods A total of 60 patients with bladder cancer were divided into control group and research group by random numerical table,with 30 cases in each group.The control group received traditional open surgery,and the research group received transurethral resection of the bladder tumor.The surgery,hospitalization indicators,postoperative visual analogue scale(VAS)scores,postoperative complications,and postoperative recurrence were compared between the two groups.Results The intraoperative blood loss in the research group was(52.29±10.53)ml,which was less than(94.39±28.67)ml in the control group,and the difference was statistically significant(P<0.05).In the research group,the operation time was(45.31±7.92)min,the extubation time was(2.14±1.03)d,and the hospitalization time was(7.03±1.17)d,which were shorter than(77.71±17.51)min,(4.57±1.65)d,and(13.64±3.55)d in the control group,and the differences were statistically significant(P<0.05).The postoperative VAS score(2.67±0.98)points of the research group was lower than(4.36±1.15)points of the control group,and the difference was statistically significant(t=6.126,P=0.000<0.05).There was no statistically significant difference in postoperative complication rates between the two groups(χ^(2)=1.964,P=0.161>0.05).The difference in postoperative recurrence rate between the two groups was not statistically significant(χ^(2)=0.351,P=0.554>0.05).Conclusion Transurethral resection of bladder tumor can achieve the same therapeutic effect as open surgery in the treatment of bladder cancer,but the former has the advantage of minimally invasive,lightpostoperative pain,early discharge and quick recovery.
