病程及年龄对单侧梅尼埃病患者听-前庭功能的影响

Effects of Disease Duration and Age on Audio-vestibular Function in Patients with Unilateral Definitive Ménière’s Disease

目的探讨病程及年龄是否影响梅尼埃病(Meniere’s disease,MD)患者的听-前庭功能,进一步了解听-前庭功能在MD评价中的价值。方法回顾性分析100例单侧MD患者的临床资料,包括:年龄、病程时间等,所有患者均进行纯音听阈(并按照临床指南进行分期)、声导抗、冷热试验、视频头脉冲试验检查;部分患者完善耳蜗电图、甘油实验检查。分析年龄和病程对患者听-前庭功能的影响。结果(1)病程对单侧MD患者的临床分期(F=2.845,P=0.042)和冷热试验异常程度(F=11.438,P<0.05)有影响,而对耳蜗电图(t=0.968,P=0.356)、甘油实验(t=0.79,P=0.435)、以及vHIT结果无影响(Z=-1.61,P=0.107);(2)年龄对单侧MD患者的临床分期有影响(H=16.289,P=0.001),而对耳蜗电图(Z=-0.796,P=0.426)、甘油实验(Z=-0.294,P=0.769)、冷热实验异常程度(H=1.044,P=0.593)以及vHIT结果(Z=-0.883,P=0.377)无影响。结论病程和年龄对单侧MD患者的听-前庭功能可造成持续影响。除控制眩晕症状外,临床上还应关注梅尼埃病患者的听力损伤情况。

Objective To investigate whether disease duration and age affect the audio-vestibular function in patients with unilateral definitive Ménière’s disease(MD)and to further understand the value of audio-vestibular function in the evaluation of MD.Methods Clinical data of 100 patients with unilateral MD were retrospectively analyzed,including:age,disease duration,etc.All patients received pure tone audiometry tests(with their conditions staged according to guidelines),caloric test,and video head impulse test(vHIT).Some patients also received electrocochleargraphy(EcochG)and glycerol tests.The correlations between age and disease duration and audio-vestibular function were analyzed.Results Disease duration affected disease stage(F=2.845,P=0.042)and the degree of caloric test abnormality(F=11.438,P<0.05),but not EcochG(t=0.968,P=0.356),glycerol test(t=0.79,P=0.435)or vHIT(Z=-1.61,P=0.107)results.Age had an effect on disease staging(H=16.289,P=0.001)and no effect on EcochG(Z=-0.796,P=0.426),glycerol test(Z=-0.294,P=0.769),caloric test abnormality(H=1.044,P=0.593)or vHIT(Z=-0.883,P=0.377)results.Conclusion Disease duration and age may have a sustained effect on audio-vestibular function in patients with unilateral MD.In addition to controlling vertigo symptoms,attention should be paid to chronic hearing impairment in these patients.
