西药房药品管理与发放过程中的问题与应对策略研究

Problems and Countermeasures in the Process of Drug Management and Distribution in Western Pharmacy

目的探讨西药房药品管理与发放差错的原因,为制定有效的应对策略提供依据。方法对我院2020年1月至2021年1月西药房药品管理及发放差错时间进行回顾,分析差错事件类型、发生原因,并提出针对性解决措施。结果2020年1月至2021年1月共发药112321次,出现药品差错113次,发生率为0.10%。药品发放差错类型主要包括剂量差错(27.43%)、用药频率差错(22.12%)、用药品种差错(17.70%)、包装规格差错(17.70%)、重复用药(11.50%)、其他(3.54%)。结论药品安全问题关乎因患者生命安全以及医院形象和利益,鼓励开具内差记录单、提升药师业务水平、强化其责任感,合理储存药品、改善药房空间布局、强化对不合理处方的审核和干预等措施可在较大程度上减少调剂差错,确保患者用药安全性与合理性,有利于建立和谐医患关系、推动医院发展。

Objective To explore the causes of drug management and distribution errors in Western pharmacy,and to provide basis for formulating effective coping strategies.Methods The time of drug management and distribution errors in our hospital from January 2020 to January 2021 were reviewed,the types and causes of error events were analyzed,and the corresponding solutions were put forward.Results From January 2020 to January 2021,a total of 112321 times of drugs were issued,113 times of drug errors occurred,and the incidence rate was 0.10%.The main types of drug distribution errors were dose error(27.43%),medication frequency error(22.12%),medication type error(17.70%),packaging specification error(17.70%),repeated medication(11.50%)and others(3.54%).Conclusion The problem of drug safety is related to the life safety of patients and the image and interests of the hospital.To encourage the issuance of internal error records,improve the professional level of pharmacists,strengthen their sense of responsibility,reasonably store drugs,improve the spatial layout of pharmacies,strengthen the audit and intervention of irrational prescriptions and other measures can reduce dispensing errors to a large extent,ensure the safety and rationality of patients’medication,which is of great significance It is beneficial to establish harmonious doctor-patient relationship and promote the development of hospital.
