柴胡龙牡汤加减联合I125放射性粒子植入治疗非小细胞肺癌的临床研究

摘要 目的观察柴胡龙牡汤加减联合I125放射性粒子植入治疗非小细胞肺癌(NSCLC)的效果与安全性。方法将我院收治的84例中晚期NSCLC患者随机分为观察组与对照组各42例。对照组采用I125放射性粒子植入方法治疗,观察组在对照组的治疗基础上加用柴胡龙牡汤加减,比较两组患者治疗前后实体瘤疗效、卡氏功能状态(KPS)评分、生活质量SF-36评分、血清癌胚抗原(CEA)、免疫功能指标,统计治疗期间患者不良反应发生率。结果治疗后,两组患者实体瘤比较差异不明显(P>0.05);观察组治疗后2个月CEA变化率高于对照组(P<0.05);观察组免疫功能指标CD3^(+)、CD4^(+)水平高于对照组,CD8^(+)水平低于对照组(P<0.05);观察组KPS评分与生活质量SF-36评分均高于对照组(P<0.05);观察组不良反应发生率低于对照组(P<0.05)。结论在I125放射性粒子植入的基础上联合使用柴胡龙牡汤加减治疗可有效提高NSCLC患者的免疫力,降低相应肿瘤标志物CEA水平,减轻治疗过程中毒副反应,提高生活质量水平。 Objective To observe the curative effect and safety of Chaihu Longmu Decoction combined with modified I125 radioactive seed implantation on non-small cell lung cancer(NSCLC).Methods Eighty-four patients with advanced NSCLC were randomly divided into observation group and control group,42 cases in each group.Control group was treated with I125 radioactive seed implantation,while observation group was additionally treated with modified Chaihu Longmu Decoction.The curative effect on solid tumors,scores of Karnofsky physical status(KPS)and Medical Outcomes Study ShortForm 36(SF-36),serum carcinoembryonic antigen(CEA)and immune function indexes before and after treatment were compared between the two groups.The incidence of adverse reactions during treatment was statistically analyzed.Results After treatment,solid tumor treatment as no significant difference in response rate of treatment between the two groups(P>0.05);after 2 months of treatment,the CEA change rate of observation group was higher than that in control group(P<0.05);the levels of immune function indexes(NK,CD3^(+),CD4^(+))of observation group were higher than those in control group,and CD8;level was lower than that in control group(P<0.05);the scores of KPS and SF-36 in observation group were higher than those in control group(P<0.05);the incidence of adverse reactions in observation group was lower than that in control group(P<0.05).Conclusion The adjuvant application of modified Chaihu Longmu Decoction based on I125 radioactive seed implantation can effectively improve immunity of NSCLC patients,reduce level of the corresponding tumor marker CEA,alleviate side effects during treatment,and improve the quality of life.
