加减二陈汤联合化痰膏穴位贴敷治疗痰哮证疗效观察及对患儿肺功能、炎性因子、免疫指标的影响

Efficacy of Modified Erchen Decoction combined with acupoint application of Huatan Ointment on children with phlegm-asthma syndrome and its influence on lung function,inflammatory factors and immune indicators

目的观察加减二陈汤联合化痰膏穴位贴敷治疗痰哮证患儿疗效及对肺功能、炎性因子、免疫指标的影响。方法将我院收治的160例痰哮证小儿哮喘患儿随机分为对照组与观察组各80例。对照组采用西药基础治疗联合化痰膏穴位贴敷,观察组在对照组治疗基础上联合加减二陈汤治疗。比较治疗前后两组中医临床症状积分、肺功能、炎性因子、免疫指标。结果治疗后,两组中医临床症状积分较治疗前明显降低( P<0.05),且观察组低于对照组( P<0.05);两组潮气量(VT)、达峰时间比(TPTEF/TE)、达峰容积比(VPTEF/VE)、功能残气量(FRC)肺功能指标均明显升高( P<0.05),且观察组高于对照组( P<0.05);两组肿瘤坏死因子(TNF-α)、白介素-6(IL-6)、白介素-18(IL-18)、免疫球蛋白G(IgG)、免疫球蛋白M(IgM)、血清补体C3免疫指标均明显降低( P<0.05),且观察组低于对照组( P<0.05)。结论加减二陈汤联合化痰膏穴位贴敷治疗痰哮证患儿疗效显著,可有效改善患儿临床症状和肺功能,降低患儿体内炎症反应,值得临床推广。

Objective To observe the efficacy of Modified Erchen Decoction combined with acupoint application of Huatan Ointment on children with phlegm-asthma syndrome and its influence on lung function,inflammatory factors and immune indicators. Methods A total of 160 children with asthma of phlegm-asthma syndrome were randomly divided into control group and observation group,with 80 cases in each group. Control group was treated with basic western medicine combined with acupoint application of Huatan Ointment,and observation group was combined with Modified Erchen Decoction on the basis of control group. The symptoms scores,lung function,inflammatory factors and immune indicators were compared between the two groups beforeand after treatment. Results After treatment,the scores of clinical symptoms of wheezing,coughing and expectoration in the two groups were significantly decreased compared with those before treatment( P<0.05),and were significantly lower in observation group than control group( P<0.05);lung function indicators oftidal volume(VT),time to tidal peak expiratory flow/expiratory time(TPTEF/TE),fraction of tidal volume exhaled at peak tidal expiratory flow to total tidal volume (VPTEF/VE)and functional residual capacity(FRC)were significantly increased in both groups compared to before treatment( P<0.05),and were significantly higher in observation group than control group( P<0.05);the levels of inflammatory factors of tumor necrosis factor(TNF-α),interleukin-6(IL-6),IL-18 and immune indicators of immunoglobulin G(IgG),immunoglobulin M(IgM)and serum complement C3 in both groups were significantly lower than those before treatment( P<0.05),and were significantly lower in observation group than control group( P<0.05). Conclusion Modified Erchen Decoction combined with acupoint application of Huatan Ointment has a significant efficacy in treatment of children with phlegm-asthma syndrome,and can effectively improve the clinical symptoms and lung function and reduce the inflammatory response,thus it is worthy of clinical promotion.
