血塞通联合耳内镜下鼓膜修补术治疗化脓性中耳炎临床疗效观察

Clinical observation on Xuesaitong combined with endoscopic tympanic membrane repair on suppurative otitis media

目的观察血塞通注射液联合耳内镜下鼓膜修补术对慢性化脓性中耳炎(CSOM)的临床疗效。方法将我院收治的68例CSOM患者随机数字表法分为观察组与对照组各34例。对照组患者采用耳内镜下鼓膜修补术治疗,观察组患者在耳内镜下鼓膜修补术的基础上采用血塞通注射液治疗,比较两组患者的疗效、治疗前后中医证候积分、炎性因子的表达及不良反应的发生率。结果观察组总有效率高于对照组(P<0.05)。治疗后两组患者耳痛、耳闷、听力下降、鼓膜内陷、中耳积液积分均下降(P<0.05),且观察组患者上述积分均低于对照组(均P<0.05)。治疗后两组两组患者血小板活化因子(PAF)、降钙酶原(PCT)、白介素(IL-1β、IL-8)表达均下降(均P<0.05),且观察组PAF、PCT、IL-1β、IL-8表达均低于对照组(均P<0.05)。观察组不良反应发生率2.94%(1/34)低于对照组26.47%(9/34)(P<0.05)。结论血塞通注射液联合耳内镜下鼓膜修补术对CSOM患者的临床疗效较显著,能明显改善中医症状,降低炎性因子的表达及不良反应的发生率。

Objective To observe the clinical efficacy of Xuesaitong Injection combined with tympanic membrane repair under otoscope on chronic suppurative otitis media(CSOM).Methods A total of 68 patients with CSOM were randomly divided into observation group and control group with 34 cases each.Control group was treated with endoscopic tympanic membrane repair,and observation group was treated with Xuesaitong Injection on the basis of control group.The curative effect,TCM syndrome scores before and after treatment,and inflammation expression were compared between the two groups.ResultsThe total effective rate of observation group was higher than control group(P<0.05).After treatment,both groups had earache,ear fullness,hearing loss,and tympanic membrane;the scores of middle ear effusion were all decreased(P<0.05),and the scores of earache,ear fullness,hearing loss,tympanic membrane and middle ear effusion of observation group were lower than those of control group(P<0.05);after treatment,the expressions of platelet activating factor(PAF),procalcitonin(PCT)and interleukin-1β(IL-1β),IL-8 in both groups decreased(P<0.05),and the expressions in observation group were lower than those in control group(P<0.05);the incidence of adverse reactions in observation group was 2.94%(1/34),lower than control group26.47%(9/34)(P<0.05).Conclusion Xuesaitong Injection combined with otoscope tympanic membrane repair has a significant clinical effect on patients with CSOM,which could significantly improve the TCM symptoms,reduce the expression of inflammatory factors and the incidence of adverse reactions.
