摘要
目的:探讨芒硝联合乙醇湿敷对维持性血液透析(MHD)静脉内瘘穿刺后血肿消退、疼痛及血管硬化程度的影响。方法:109例MHD动静脉内瘘穿刺后血肿患者按随机数字表法分为两组。对照组54例应用传统冷热疗法护理,观察组55例应用芒硝联合95%乙醇湿敷护理,观察两组的干预效果。结果:观察组总有效率92.73%高于对照组77.78%,差异有统计学意义(P<0.05);血肿消退时间、淤斑消退时间、血肿直径短于对照,血管硬化程度低于对照组(P<0.05);干预后24 h,观察组疼痛程度低于对照组(P<0.05);两组并发症发生率的差异无统计学意义(P>0.05)。结论:芒硝联合乙醇湿敷可有效消退MHD动静脉内瘘穿刺后患者的血肿,降低穿刺后的疼痛,提高干预效果。
Objective: To investigate the effects of mirabilite combined with alcohol wet compress on hematoma regression, pain and degree of vascular sclerosis in maintenance hemodialysis(MHD)patients with hematoma after intravenous fistula puncture. Methods: 109 MHD patients with hematoma after intravenous fistula puncture were randomized into two groups. 54 cases in the control group were treated with traditional cold and heat therapy nursing, while another 55 cases in the observation group were treated with mirabilite combined with 95% alcohol wet compress nursing. Then, the clinical efficacy was compared between the two groups. Results: The total effective rate of the observation group was 92.73%, which was higher than 77.78% in the control group, which was of statistical difference(P<0.05);hematoma and ecchymosis regression time, hematoma diameter, and degree of vascular sclerosis were all lower than those in the control group(P<0.05);24 h after intervention, the pain degree in the observation group was lighter than that in the control group(P<0.05);and there was no significant difference in the incidence of complications between the two groups(P>0.05). Conclusion: Mirabilite combined with alcohol wet compress can effectively reduce the hematoma and the pain, and help to improve the effect in MHD patients after arteriovenous fistula puncture.
