摘要
目的探究宫颈癌及癌前病变组织中膜联蛋白A8(ANXA8)、基质金属蛋白酶2(MMP2)的表达及临床意义。方法收集本院病理科存档的宫颈石蜡组织标本170例,其中正常宫颈组织30例,宫颈上皮内病变组织70例,宫颈癌组织70例,利用癌症基因组图谱(TCGA)查询ANXA8、MMP2基因在宫颈癌中的表达水平,采用免疫组织化学法检测ANXA8、MMP2蛋白在3种组织中的表达情况,分析ANXA8、MMP2蛋白水平与宫颈癌患者临床病理资料的关系,分析ANXA8、MMP2蛋白在3种组织中的表达相关性,利用受试者操作特征(ROC)曲线分析ANXA8、MMP2蛋白表达阳性对宫颈癌的诊断价值。结果 ANXA8、MMP2蛋白在宫颈癌、宫颈上皮内病变、正常宫颈组织中的阳性表达率依次显著降低(P<0.05),其表达水平与肿瘤组织分化程度、FIGO分期、淋巴结转移有关(P<0.05);相关性分析显示,ANXA8蛋白在宫颈癌、宫颈上皮内病变及正常宫颈组织中的表达与MMP2蛋白的异常表达均有关(P<0.05);ROC结果显示,ANXA8、MMP2蛋白阳性表达诊断宫颈癌的曲线下面积分别为0.749(95%CI:0.677~0.812)、0.783(95%CI:0.713~0.842),约登指数分别为0.498、0.566。结论 ANXA8、MMP2在宫颈癌组织、癌前病变组织及正常宫颈组织中阳性表达率依次显著降低,其高表达与肿瘤分化、转移有关,并对宫颈癌有一定的诊断价值。
Objective To explore the expression and clinical significance of annexin A8(ANXA8) and matrix metalloproteinase 2(MMP2) in cervical cancer and precancerous lesions. Methods A total of 170 cases of cervical paraffin tissue specimens archived in the pathology department of our hospital, including 30 cases of normal cervical tissues, 70 cases of cervical intraepithelial lesion tissues, and 70 cases of cervical cancer tissues. The expression levels of ANXA8 and MMP2 genes in cervical cancer were queried with The Cancer Genome Atlas(TCGA), the expression of ANXA8 and MMP2 protein in three tissues was detected with immunohistochemical method, the relationship between ANXA8, MMP2 protein levels and clinicopathological data of cervical cancer patients was analyzed, the correlation of ANXA8 and MMP2 protein expression in three tissues was analyzed, the diagnostic value of ANXA8 and MMP2 protein expression for cervical cancer was analyzed with receiver operating characteristic curve(ROC). Results The positive expression rates of ANXA8 and MMP2 proteins in cervical cancer, cervical intraepithelial lesions, and normal cervical tissues were significantly decreased in turn(P<0.05), and their expression levels were correlated with the degree of tumor tissue differentiation, FIGO staging, and lymph node metastasis(P<0.05);correlation analysis showed that the expression of ANXA8 protein in cervical cancer, cervical intraepithelial lesions and normal cervical tissues was related to the abnormal expression of MMP2 protein(P<0.05);ROC results showed that the area under the curve of the positive expression of ANXA8 and MMP2 proteins for the diagnosis of cervical cancer was 0.749(95%CI: 0.677–0.812) and 0.783(95%CI: 0.713–0.842), respectively, and the Youden index was 0.498 and 0.566, respectively. Conclusion The positive expression rates of ANXA8 and MMP2 in cervical cancer tissues, precancerous lesion tissues and normal cervical tissues decrease significantly in turn. Their high expression is related to tumor differentiation and metastasis, and has certain diagnostic value for cervical cancer.
