摘要
目的:研究改良国际卵巢癌分析组织(IOTA)简单法则联合超声造影诊断卵巢肿块良恶性的价值。方法:将医院2018年9月—2020年12月间,常规超声检查疑似恶性病变的107例卵巢肿块患者纳为研究对象,以病理诊断结果作为“金标准”,分析超声造影及改良IOTA简单法则在诊断卵巢肿块良恶性中的价值。结果:改良IOTA简易法则在评估卵巢肿块良恶性中的准确率为68.22%,其中有20例患者不能经改良IOTA简易法则区分肿块性质,12例误诊,2例漏诊。超声造影在评估卵巢肿块性质中的灵敏度、特异度及准确度分别为84.91%、81.48%和83.18%,其中8例误诊,10例漏诊。超声联合改良IOTA简易法则在区分卵巢肿块良恶性中的灵敏度、特异度及准确度分别为96.23%、92.59%和94.39%,其中2例误诊,4例漏诊。结论:改良IOTA简易法则与超声造影在评估卵巢肿块性质中均具有一定的价值,而两种方法相联合可有效弥补其单独应用时的不足,提高卵巢肿块良恶性鉴别效果。
Objective:To investigate the value of modified international ovarian tumor analysis(IOTA)simple rules combined with contrast-enhanced ultrasound in differential diagnosis of benign and malignant ovarian masses.Methods:A total of 107 patients with suspected malignant ovarian masses diagnosed by conventional ultrasound in our hospital from September 2018 to December 2020 were included.Taking pathological diagnosis results as"gold standard",the value of contrast-enhanced ultrasound and modified IOTA simple rule in the diagnosis of benign and malignant ovarian masses was verified.Results:The accuracy rate of modified IOTA simple rule in the differentiation diagnosis of benign and malignant ovarian masses was 68.22%,including 20 unclassified cases,12 misdiagnosed cases,and 2 miss diagnosed cases.The sensitivity,specificity and accuracy of contrast-enhanced ultrasound in the evaluation of ovarian masses were 84.91%,81.48%and 83.18%,respectively.Among them,8 cases were misdiagnosed and 10 cases were miss diagnosed.Conclusion:Both modified IOTA simple rules combined with contrast-enhanced ultrasound have certain value in assessing the characteristics of ovarian masses,and the combination of the two can effectively improve the accuracy of clinical diagnosis.
