复方百蛇洗剂治疗湿毒瘀结型宫颈人乳头瘤病毒感染的疗效及对血清炎症因子和免疫因子的影响

Curative effect of compound Baishe lotion for cervical human papillomavirus infection of dampness-toxin stasis type and how it impacts on serum inflammatory factor and local immune factor

目的观察复方百蛇洗剂治疗湿毒瘀结型宫颈人乳头瘤病毒(HPV)感染的疗效及对血清炎症因子和免疫因子的影响。方法将80例湿毒瘀结型宫颈HPV感染患者按照随机数字表法分为2组,对照组40例予重组人干扰素α2b阴道泡腾胶囊治疗,治疗组40例在对照组治疗基础上加复方百蛇洗剂治疗,2组均连续治疗2个月。比较2组疗效;观察2组治疗前后炎症因子白细胞介素1β(IL-1β)、肿瘤坏死因子β(TNF-β)、IL-23、IL-17变化;观察2组治疗前后免疫因子CD3^(+)、CD4^(+)、CD8^(+)、调节性T淋巴细胞(Treg)、辅助性T淋巴细胞17(Th17)、自然杀伤(NK)细胞水平变化;比较2组治疗前后中医证候评分变化;记录2组HPV-DNA转阴率。结果治疗组总有效率92.50%(37/40),对照组总有效率70.00%(28/40),治疗组疗效优于对照组(P<0.05)。2组治疗后IL-1β、TNF-β、IL-23、IL-17水平均较本组治疗前降低(P<0.05),且治疗组降低更明显(P<0.05)。2组治疗后CD3^(+)、CD4^(+)、NK细胞水平均较本组治疗前升高(P<0.05),CD8^(+)、Th17、Treg水平均较本组治疗前降低(P<0.05);治疗后治疗组CD3^(+)、CD4^(+)、NK细胞水平均高于对照组(P<0.05),CD8^(+)、Th17、Treg水平均低于对照组(P<0.05)。2组治疗后中医证候各项评分均较本组治疗前降低(P<0.05),且治疗组降低更明显(P<0.05)。治疗组HPV-DNA转阴率90.00%(36/40),对照组HPV-DNA转阴率67.50%(27/40),治疗组HPV-DNA转阴率高于对照组(P<0.05)。结论复方百蛇洗剂治疗湿毒瘀结型宫颈HPV感染,可降低患者血清炎症因子水平,调节免疫因子水平,改善患者临床症状,提高患者HPV-DNA转阴率。

Objective To observe the curative effect of compound Baishe lotion for cervical human papillomavirus(HPV)infection of dampness-toxin stasis type and how it impacts on serum inflammatory factor and local immune factor.Methods Totally 98 patients with cervical HPV infection(dampness-toxin stasis type)were assigned into equal group according to the random number table,in addition to recombinant human interferonα2b vaginal effervescent capsules,patients in treatment group additionally received compound Baishe lotion.The treatment continued 2 months to compare inflammatory factors[interleukin-1 beta(IL-1β),tumor necrosis factor-β(TNF-β),IL-23,IL-17],local immune factor[CD3^(+),CD4^(+),CD8^(+),regulatory T cells(Treg),T helper cell 17(Th17),natural killer cell(NK)],traditional Chinese medicine(TCM)syndrome score;the curative effect and HPV-DNA negative conversion rate were counted.Results The total effective rates in treatment group were better than those in control group[92.50%(37/40)vs 70.00%(28/40),(P<0.05),respectively].After treatment,IL-1β,TNF-β,IL-23,IL-17 in groups were decreased(P<0.05),which in treatment group were significantly decreased(P<0.05);CD3^(+),CD4^(+),NK in groups were increased(P<0.05),which in treatment group was higher than in control group(P<0.05);CD8^(+),Th17,Treg in groups were decreased(P<0.05),which decreased in treatment group compared with cortrol group(P<0.05);TCM symptom scores in groups were decreased(P<0.05),which were notably decreased in treatment group(P<0.05).HPV-DNA negative conversion rate was 90.00%(36/40)in treatment group,higher than that of 67.50%(27/40)in control group(P<0.05).Conclusion For patients with cervical HPV infection(dampness-toxin stasis type),compound Baishe lotion is capable of reducing the level of serum inflammatory factors,regulating local immune factors,improving clinical symptoms,and increasing HPV-DNA negative conversion rate.
